What the Costco Membership Application Form Is

The Costco Membership Application Form is the record used to enroll an individual or business in Costco membership programs, specify member type (Gold Star, Executive, Business), collect contact and payment details, and establish membership terms and renewal preferences. It captures identifying information, household card requests, and consent for membership policies. The form may be completed on paper at a warehouse, submitted online, or signed electronically; when executed electronically it must meet U.S. e-signature legal standards such as the ESIGN Act (15 U.S.C. ch. 96) and applicable state UETA provisions.

Why the Application Matters for Members and Administrators

The form creates a binding membership record, enables payment authorization and account setup, and documents member consent to Costco terms. Accurate completion reduces processing delays and billing errors, supports household card issuance, and provides a clear audit trail when completed electronically using compliant eSignature platforms such as signNow.

Why the Application Matters for Members and Administrators

Who Completes the Costco Membership Application Form

  • Corporate Administrators: Procurement or HR staff managing multiple memberships and payment methods for employees.

Understanding the primary user helps determine which fields are required, which supporting documents to attach, and whether a business tax ID or additional authorization is needed.
