What a Counseling Services Description Covers

A Counseling Services Description is a formal written document that outlines the scope, objectives, methods, and administrative terms of counseling provided to an individual or group. It typically specifies client eligibility, session frequency and duration, confidentiality limits, data sharing and recordkeeping practices, fees or funding sources, and the roles and responsibilities of counselors and clients. Organizations use it to set expectations, document informed consent, and support billing or insurance processes. The Description may be adapted for clinical, educational, employment, or community settings while adhering to applicable laws and professional standards.

Why a Clear Description Matters for Care and Compliance

Documenting counseling services clarifies responsibilities, reduces misunderstandings, and creates a record that supports clinical decisions, billing, and compliance. A clear Description helps demonstrate informed consent, protect client privacy under HIPAA where applicable, and provide consistent service expectations across providers.
