Whereas, Defendant __________________________ , a single woman, gave birth to a __________________________ (male or female) child, since named __________________________ (Name of Child), on __________________________ (date) , at __________________________ (e.g., name and address of hospital); Whereas, Complainant __________________________ is the father of said child as shown by the Declaration of Paternity filed with the Office of Vital Records of the State of _________, a copy of which is attached hereto as Exhibit A and as shown by the Certificate of Live Birth issued by the State of __________, a copy of which is attached hereto as Exhibit B. In consideration of the matters described above, and of the mutual benefits and obligations set forth in this agreement, the parties agree as follows: I. Defendant consents to entry of judgment in accordance with the terms of this agreement. V.
