CUSTODY CONSENT AGREEMENT

This Custody Consent Agreement ("Agreement") is entered into on this by and between Parent/Guardian 1: of (hereinafter "Grantor"), and Parent/Guardian 2: of (hereinafter "Recipient").

RECITALS

WHEREAS, the parties are the parents or legal guardians of the minor child named below and have authority to execute this Agreement;

WHEREAS, the parties desire to set forth their mutual agreement regarding temporary or ongoing custody and decision-making authority with respect to the child: Child Full Name: ; Date of Birth:

WHEREAS, Grantor wishes to grant and Recipient wishes to accept custody and authority subject to the terms and conditions set forth herein for the welfare, care, and protection of the child.

NOW, THEREFORE, in consideration of the mutual covenants contained herein and other good and valuable consideration, the parties agree as follows:

1. DEFINITIONS

For purposes of this Agreement, the following terms shall have the following meanings: "Child" means the individual identified above; "Grantor" means the party identified as Grantor; "Recipient" means the party identified as Recipient; "Custody" means physical care, control, and authority to make routine and emergency decisions regarding the Child's health, education, and welfare as provided herein.

2. GRANT OF CUSTODY AND CONSENT

Grantor hereby voluntarily and knowingly grants to Recipient custody of the Child, including authority to make day-to-day decisions concerning the Child's care and custody. This grant includes authority as set forth in this Agreement and is given: Temporary Permanent If Temporary, Term Duration (if applicable):

3. SCOPE OF AUTHORITY

Recipient is authorized to exercise the following powers on behalf of the Child as necessary for the Child's care:

Make medical and dental decisions, including consenting to examinations and treatment
Enroll the Child in school and make educational decisions
Authorize domestic and international travel on behalf of the Child (subject to Clause 6)
Provide day-to-day care and discipline appropriate to the Child's age

4. LIMITATIONS ON AUTHORITY

Recipient shall not, without the prior written consent of Grantor: (a) consent to adoption of the Child; (b) permanently relocate the Child outside the geographic area specified below; or (c) make any long-term change in legal custody. Geographic limitation (if any):

5. MEDICAL CARE AND EMERGENCIES

Grantor authorizes Recipient to consent to medical, dental, psychiatric, and surgical treatment for the Child in the event of an emergency or when such treatment is reasonably necessary for the Child's health and safety. Recipient shall make reasonable efforts to notify Grantor prior to non-emergency treatment.

6. TRAVEL, PASSPORTS, AND INTERNATIONAL MOVEMENT

Recipient may apply for and obtain travel documents, including passport and visas, for the Child only as follows:

7. TERM, REVOCATION, AND NOTICE

This Agreement shall become effective on the date set forth above and shall remain in effect until the earlier of: (a) the expiration date specified by the parties; (b) written revocation by Grantor delivered in accordance with the Notices provision; or (c) a court order modifying or terminating custody.

8. REPRESENTATIONS, WARRANTIES, AND AUTHORITY

Each party represents and warrants that: (a) such party has full legal capacity and authority to enter into this Agreement; (b) execution of this Agreement does not violate any court order, existing agreement, or law; and (c) there are no outstanding custody orders or proceedings that would render this Agreement void or unenforceable other than those disclosed below.

9. INDEMNIFICATION

Grantor agrees to indemnify and hold harmless Recipient from and against any claims, liabilities, losses, or expenses (including reasonable attorneys' fees) arising out of Recipient's good faith performance of duties under this Agreement, except to the extent resulting from Recipient's willful misconduct or gross negligence.

10. NOTICES

All notices required or permitted under this Agreement shall be in writing and delivered personally, by certified mail return receipt requested, or by nationally recognized overnight courier to the addresses below or such other address as a party may designate by written notice.

11. AMENDMENT; WAIVER; COUNTERPARTS

This Agreement may be amended only by a written instrument signed by both parties. No waiver of any breach shall be deemed a waiver of any subsequent breach. This Agreement may be executed in counterparts, each of which shall be deemed an original and all of which together shall constitute one instrument.

12. GOVERNING LAW; ENTIRE AGREEMENT; SEVERABILITY

This Agreement shall be governed by and construed in accordance with the laws of the state specified by the parties below. This Agreement constitutes the entire agreement between the parties with respect to its subject matter and supersedes all prior agreements and understandings. If any provision of this Agreement is found invalid or unenforceable, the remaining provisions shall continue in full force and effect.

13. EXECUTION AND ACKNOWLEDGMENT

Each party acknowledges that they have read this Agreement, understand its terms, and execute it voluntarily. Each party further acknowledges that they are not under duress or undue influence and that execution of this Agreement is intended to be a binding legal act.
