What the Custom Sticker Request Form Is

A Custom Sticker Request Form is a structured document used to collect client instructions, artwork files, production specifications, billing data, and approvals needed to manufacture or order bespoke stickers. It standardizes the information vendors require—dimensions, materials, quantities, colors, finish, and delivery — to produce accurate proofs and reduce back-and-forth during prepress. For organizations that accept remote orders, the form can be completed and executed electronically, preserving an auditable record of client intent, acceptance of terms, and file attachments required for production and invoicing.

Why a Formal Request Form Matters

Using a standardized Custom Sticker Request Form reduces errors, shortens production cycles, and creates a clear record of client instructions and approvals. It helps allocate responsibility for artwork accuracy and provides an audit trail for disputes.

Why a Formal Request Form Matters

Who Typically Completes This Form

The form is completed by anyone requesting custom stickers on behalf of a company, project, or campaign.

  • Marketing managers submitting campaign-approved artwork and delivery dates.
  • Procurement or purchasing agents consolidating multiple orders for volume pricing.
  • Individual customers or store managers ordering branded labels for retail or events.

Signers may include authorized representatives, procurement officers, or individual account holders depending on internal approval workflows.

Elements of a professional Custom Sticker Request Form

A well-constructed form groups essential data into clear sections to speed approvals and support compliance, invoicing, and production planning.

Request header

Includes requester name, company, contact phone, email, purchase order number, and billing contact to link the job to accounting and reduce back-and-forth.

Product specs

A dedicated section for material type, thickness, size (W x H), corner radius, lamination, adhesive type, and finish ensures accurate quoting and production setup.

Artwork and proofing

File upload area with instructions for bleed, color profile, resolution, and preferred proofing method prevents rework and clarifies expectations.

Quantities & pricing

Per-SKU quantity fields, tiered pricing indicators, and requested delivery dates help procurement and production schedule capacity and cost estimates.

Legal and IP

A short warranty and intellectual property confirmation assigns responsibility for trademark clearance and grants the vendor limited rights to reproduce supplied artwork.

Signatures & approvals

Designated signature blocks for requester and approver with dates and eSignature audit data confirm acceptance and authorization to proceed.

Common errors that delay sticker orders

  • Uploading non-print-ready artwork leads to automatic rejections and proof delays.
  • Omitting full specifications (material or finish) forces vendor follow-up and schedule slips.
  • Incorrect quantities or unit mismatches cause invoicing and production errors.
  • Missing approval signatures or unclear approver authority halts production until clarified.

Risks of incomplete or incorrect requests

Production delay: Delayed shipping
Extra charges: Reprint or rush fees
Legal exposure: IP infringement claims
Order cancellation: Rejected artwork
Billing disputes: Incorrect invoice amounts
Data privacy: HIPAA exposure risk

Typical timelines and deadlines to set expectations
