DANCE TEAM AGREEMENT

This Dance Team Agreement ("Agreement") is entered into as of by and between Organization Name: whose principal address is (hereinafter "Organization"), and Participant Name: of (hereinafter "Participant").

RECITALS

WHEREAS, Organization operates a dance team program that provides instruction, rehearsals, performances and related activities; and

WHEREAS, Participant desires to participate as a member of the dance team and Organization is willing to accept Participant on the terms and conditions set forth herein; and

WHEREAS, the parties intend to define their respective rights, duties, and obligations with respect to membership, fees, conduct, and liability.

NOW, THEREFORE

In consideration of the mutual covenants contained herein and other good and valuable consideration, the receipt and sufficiency of which are acknowledged, the parties agree as follows:

1. TERM

The term of this Agreement shall commence on the Effective Date set forth above and shall continue until terminated in accordance with Section 12. The initial active season is described as:

2. MEMBERSHIP; ELIGIBILITY

Participant represents and warrants that Participant meets all eligibility requirements established by Organization, including but not limited to age, skill level, and any required clearances. Participant shall promptly notify Organization of any change in eligibility.

3. DUTIES AND OBLIGATIONS

Participant agrees to attend scheduled rehearsals, performances, competitions, and promotional events as directed by Organization. Participant shall comply with all policies, rules, and code of conduct established by Organization. Repeated unexcused absences or conduct detrimental to the team may result in suspension or termination.

4. PRACTICES, PERFORMANCES, AND TRAVEL

Participant acknowledges that participation may require travel and irregular hours. Organization will provide schedules in advance when reasonably practicable. Participant shall adhere to curfews, safety protocols and supervision rules during travel. Organization is not required to provide individual chaperones except as may be required by applicable law or Organization policy.

5. FEES, PAYMENT, AND EXPENSES

Participant shall pay fees as consideration for instruction, coaching, travel, and administrative costs. The fee amount for the season is payable in accordance with the following schedule:

Late payments may incur a late fee and failure to pay may result in suspension of privileges. Participant remains responsible for fees incurred prior to any termination.

6. UNIFORMS, EQUIPMENT AND PROPERTY

Organization will provide or require purchase of uniforms and certain equipment. Participant shall maintain uniforms in good condition and return Organization property upon request. Costs for replacement due to loss, theft, or damage caused by Participant beyond reasonable wear and tear shall be borne by Participant.

7. MEDICAL AUTHORIZATION AND LIABILITY RELEASE

Participant certifies that Participant is physically fit to participate in dance activities. Participant authorizes Organization to obtain emergency medical treatment if Participant is injured or becomes ill while participating. Participant hereby releases and holds harmless Organization, its officers, directors, employees, agents and volunteers from any claims, liabilities, damages or expenses arising out of or related to Participant's participation except to the extent caused by Organization's gross negligence or willful misconduct.

8. CODE OF CONDUCT; DISCIPLINE

Participant shall conduct themselves in a professional manner that reflects positively on Organization. Prohibited conduct includes but is not limited to theft, intoxication, use of illegal substances, harassment, and actions endangering other members. Organization may impose disciplinary measures including warnings, suspension, reassignment of roles, or termination in accordance with established disciplinary procedures.

9. INTELLECTUAL PROPERTY; MEDIA RELEASE

All choreography, routines, recordings, photographs and other creative works developed or used by Organization in the course of the team shall remain the property of Organization. Participant grants Organization a perpetual, royalty-free right to use Participant's name, image, voice and performance in connection with promotional materials, recordings, broadcasts and social media. Participant waives any claim for compensation for such uses.

10. TERMINATION AND SUSPENSION

Either party may terminate this Agreement for material breach by the other party if such breach is not cured within fourteen (14) days after written notice. Organization may suspend or remove Participant immediately for serious misconduct or safety concerns. Termination does not relieve Participant of obligations accrued prior to termination.

11. NOTICES

Any notice required or permitted under this Agreement shall be in writing and delivered to the addresses set forth in the opening paragraph or to such other address as either party may designate by written notice. Notices are effective upon personal delivery, confirmed electronic transmission, or three (3) days after deposit in the U.S. mail, postage prepaid.

12. AMENDMENTS AND WAIVER

No amendment or modification of this Agreement shall be effective unless in writing and signed by both parties. Waiver of any breach or default shall not constitute a waiver of any subsequent breach or default.

13. GOVERNING LAW

This Agreement shall be governed by and construed in accordance with the laws of the state of without regard to conflict of laws principles.

14. ENTIRE AGREEMENT; SEVERABILITY; COUNTERPARTS

This Agreement constitutes the entire agreement between the parties concerning its subject matter and supersedes all prior agreements and understandings. If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect. This Agreement may be executed in counterparts, each of which shall be deemed an original, and all of which together shall constitute one and the same instrument.

ADDITIONAL ACKNOWLEDGMENTS

Participant acknowledges receipt of Organization rules and policies and agrees to comply with them. Participant further acknowledges that participation involves inherent risks and that they have had an opportunity to ask questions and obtain clarification regarding those risks.
