What the Data Disposal Request Form Is

A Data Disposal Request Form documents a formal request to dispose of or destroy specific records, data sets, or physical media in accordance with internal policy and applicable law. It identifies the data owner, scope of records, method of disposal, and required approvals, creating an auditable trail for compliance and governance. Organizations use the form to authorize secure deletion, schedule destruction, and record who carried out each step. Properly completed forms reduce regulatory risk and demonstrate that disposal followed documented procedures and retention schedules.

Why a Formal Disposal Request Matters

Using a standardized Data Disposal Request Form creates an auditable record of decisions, ensures consistent handling of sensitive information, and helps meet legal retention and privacy obligations such as HIPAA and IRS rules.
