By signing this document, Cardholder Name: authorizes Processor Name: to initiate debit entries to the specified funding source in order to load funds onto the designated debit card or account. This authorization applies to the amounts and schedule set forth in the Load Details section above.

Cardholder represents and warrants that the funding account or card provided is owned or lawfully controlled by the Cardholder and that Cardholder is authorized to permit debits from that account. Cardholder agrees that the Processor may charge applicable fees disclosed at the time of the load and that such fees may be deducted from the load amount or charged separately to the funding source.

Cardholder understands that revocation of this authorization must be provided in writing and delivered to the Processor at the address provided above. Revocation will not affect transactions already initiated. Processor and any financial institutions involved are authorized to rely on this authorization until receipt of written revocation and a reasonable opportunity to act on it.

Cardholder agrees to indemnify and hold Processor harmless from any liability arising from unauthorized or fraudulent use of the funding source, except to the extent caused by Processor's gross negligence or willful misconduct. Processor reserves the right to refuse or delay any load for reasons including but not limited to suspected fraud, regulatory compliance, or funding verification.

Disputes related to debits initiated pursuant to this authorization shall be governed by the laws of the state specified by the Processor in its records, and Cardholder acknowledges receipt of all required disclosures concerning electronic debits and agrees to any applicable account rules and bank operating regulations.
