What the Delivery Request Form Is and when it's used

A Delivery Request Form is a standardized document used to arrange, authorize, and track the handoff of goods between a sender, carrier, and receiver. It typically records requester and recipient contact details, pickup and delivery addresses, item descriptions, quantities, required delivery window, special handling instructions, and signature fields. Organizations use this form to confirm logistics details, create a record for billing or claims, and provide an audit trail for chain-of-custody. The form can be completed on paper or as an electronic, fillable document with an attached audit trail to document acceptance and timestamps.

Why a clear Delivery Request Form matters

Using a complete Delivery Request Form reduces scheduling errors, clarifies responsibilities, and preserves evidence of agreed delivery terms. A well-structured form minimizes disputes, supports billing and claims processes, and speeds coordination across logistics partners.

Why a clear Delivery Request Form matters

Who prepares and signs Delivery Request Forms

Responsibilities should be assigned up front so each party knows which fields to complete and who must sign at each stage.

  • Shippers and logistics coordinators: prepare requests and confirm pickup details.
  • Carriers and drivers: accept assignments, record pickup times, and provide signatures on delivery.
  • Receiving staff and warehouse teams: verify items, note exceptions, and sign proof of delivery.

Step-by-step: completing and issuing a Delivery Request Form

Follow these steps to complete the form accurately and ensure timely processing.

  • 01
Prepare the form: Gather order numbers, addresses, item details, and any permits.
  • 02
Complete required fields: Enter contact info, pickup/delivery addresses, and item lists.
  • 03
Attach supporting documents: Add invoices, manifests, or hazardous-material data sheets if required.
  • 04
Sign and distribute: Obtain signatures and send copies to carrier and recipient.

Configure an electronic Delivery Request workflow

Set up routing, authentication, and notifications so every party receives the correct version and signs in order.

Field Configuration
Signer Order Sequential or parallel routing based on role and approval requirements
Authentication Method Email link, SMS code, or multi-factor for higher-assurance signers
Notifications Automatic email or SMS reminders and completion notices
Attachments Permit PDF, BOL, MSDS uploads and required supporting forms

Where to send the completed Delivery Request Form

Routing depends on role: the carrier, requestor, and receiving location each need copies for operations and records.

  • Carrier Dispatch: Send signed copy to the carrier's operations inbox or TMS.
  • Requestor Record: Store a PDF with audit trail in procurement or shipping records.
  • Recipient Notification: Provide a delivery notice to the receiving contact by email or SMS.
  • Archive: Retain final signed version in secure document storage for claims.

Technical and format requirements for electronic submission

Ensure the selected system captures timestamps and signer attribution, exports audit reports, and integrates with your existing platforms for routing and storage.

  • File Formats: PDF, DOCX accepted
  • Integrations: TMS, ERP, and cloud storage
  • Authentication: Email link, SMS, or SSO

  • Vague item descriptions that prevent correct vehicle assignment or hazard handling and lead to unloading delays.
  • Not specifying access restrictions or required credentials, causing loading dock denials or extended wait times.
  • Failing to attach permits or MSDS for regulated materials, which can prevent pickup or result in fines.

Risks and potential charges from incorrect or incomplete forms

Detention Fees: Carrier charges for excessive wait time
Reconsignment Costs: Fees to reroute or redirect shipments
Storage Charges: Warehouse fees for unclaimed goods
Damage Liability: Disputed damage claims without POD
Regulatory Penalties: Fines for missing hazardous-material documentation
