What a Delivery Routing Slip Is and when it's used

A Delivery Routing Slip is a structured record used to direct, track, and document the physical movement of packages, documents, specimens, or equipment between people, departments, or locations. It records sender and recipient details, instructions for handling, timestamps, and proof of transfer or receipt. Organizations use routing slips to maintain chain-of-custody, confirm delivery conditions, and provide an auditable history for compliance or billing. Typical environments include courier operations, healthcare specimen transfers, legal document handling, facilities management, and interoffice deliveries where documented handoffs reduce disputes and operational errors.

Why a clear Delivery Routing Slip matters

A well-prepared routing slip reduces misdeliveries, preserves chain-of-custody, and creates a searchable record for audits and billing. It helps recipients understand handling requirements, identifies responsible parties, and shortens resolution times when discrepancies occur.

Why a clear Delivery Routing Slip matters

Who completes and relies on routing slips

Multiple roles create, attach, or reference routing slips across a delivery lifecycle.

  • Shipping clerks and warehouse staff who prepare and dispatch items for delivery.
  • Couriers and drivers who record handoffs and obtain recipient signatures on delivery.
  • Records managers and compliance teams who retain slips for audit and dispute resolution.

Use depends on organizational structure; routing slips are practical wherever physical handoffs require documentation and accountability.

Essential parts of a professional Delivery Routing Slip

A professional routing slip combines identification, instructions, confirmation, and retention metadata so each handoff is unambiguous and verifiable.

Sender Details

Full name, department, contact phone or email, and a recognizable internal code to locate origin quickly in records.

Recipient Details

Full recipient name, department or delivery address, contact phone, and preferred delivery window to avoid failed attempts.

Delivery Instructions

Handling requirements, temperature controls, signature requirements, and any special routing steps needed to meet service level expectations.

Tracking and IDs

Unique routing or tracking number, barcode or QR code, and any carrier or internal reference to link with shipment systems.

Signatures & Times

Designated signature lines, printed name fields, and timestamp fields for each transfer to capture custody and acceptance evidence.

Internal Routing Codes

Department codes, project numbers, or invoice references used to route documentation correctly and assist with billing reconciliation.

Step-by-step: completing and using a Delivery Routing Slip

Follow these steps each time you prepare and close out a routed delivery to maintain clear custody and traceability.

  • 01
Prepare: Generate a new routing number and fill sender, recipient, and instructions.
  • 02
Attach: Affix the slip to the package or include a copy inside; add barcode or QR if available.
  • 03
Deliver: Courier performs handoff, records transfer time, and obtains recipient signature.
  • 04
Record: Return or upload completed slip to records system for retention and audit.

Typical routing workflow from dispatch to receipt

A routing slip supports a predictable flow: create, route, confirm receipt, and archive for audit or billing.

  • Create Slip: Sender completes fields and assigns routing number before dispatch.
  • Dispatch: Package departs with attached slip and tracking identifier.
  • Handoff: Courier obtains recipient signature and records time of delivery.
  • Archive: Completed slip stored per retention policy for compliance and dispute resolution.

Standard configuration settings for electronic routing slips

Configure your digital workflow to mirror paper practices while adding validation and audit capabilities.

Field Configuration
Routing Number Auto-generate sequential or UUID-based value
Required Fields Set sender, recipient, signature, and date as mandatory
Authentication Use email link, SMS code, or stronger MFA for signer identity
Retention Policy Automatically archive completed slips to secure storage

Digital distribution and system requirements

Choose a platform that supports form fields, routing, audit trail, and integrations with enterprise systems.

  • Formats Supported: PDF, DOCX, and editable HTML templates
  • Integrations: Salesforce, NetSuite, Google Workspace, Box, Procore
  • Authentication: Email link, SMS OTP, or SSO per workflow needs

Ensure the platform logs timestamps, signer attribution, and any authentication events so completed slips meet audit and compliance requirements.

Common preparation errors to avoid

  • Incomplete recipient details leading to failed delivery and costly redelivery attempts if phone or location is missing.
  • Unclear handling instructions that result in damaged goods or missed temperature controls during transit and storage.
  • Failure to capture signatures or timestamps at each handoff, weakening chain-of-custody and complicating dispute resolution.
  • Not archiving completed slips in searchable storage, making audits and billing reconciliations slow and error-prone.

Risks and potential consequences of incorrect routing slips

Late Delivery: Customer chargebacks
Loss of Evidence: Weakened dispute position
Regulatory Fines: Possible industry penalties
Chain-of-Custody Gaps: Compromised forensic value
Billing Errors: Incorrect client invoicing
Invalid Signatures: Rejected proof of delivery
