What the Diversity and Inclusion Ethnic-Racial Form Is

The Diversity and Inclusion Ethnic-Racial Form is a standardized demographic questionnaire used by employers, schools, and organizations to collect self-reported race and ethnicity data for voluntary reporting, equal employment opportunity monitoring, or internal diversity metrics. It records categories aligned with federal reporting where applicable while distinguishing between race and Hispanic/Latino ethnicity. The form is typically anonymous or linked to an employee record depending on purpose, and it should explain voluntary participation, purpose, confidentiality safeguards, and how aggregated results will be used.

Why this Form Matters for Compliance and Inclusion

Collecting consistent ethnic and racial data supports legally required reporting, diversity analysis, and targeted inclusion efforts while helping demonstrate compliance with nondiscrimination obligations under federal law such as Title VII. Using a clear, voluntary form reduces ambiguity and preserves employee privacy.
