What the Document Destruction Request Form Is

A Document Destruction Request Form is a written record used to authorize the disposal, destruction, or secure shredding of business records and physical or electronic files. It documents who requested destruction, which records are affected, the approved destruction method, and the scheduled disposal date, creating an auditable trail for compliance and internal control purposes.

Why a Formal Request Form Matters

Using a standardized form reduces accidental loss, supports legal defensibility, and creates an auditable chain of custody that satisfies internal policies and external regulators.

Why a Formal Request Form Matters

Who Typically Completes a Destruction Request

These request forms are completed by staff who control or custody records and by authorized approvers before disposal.

  • Records managers and compliance officers who maintain retention schedules and legal holds.
  • Department supervisors approving disposal for employee or project records within their area.
  • Third-party vendors or facilities requests when physical media are returned for destruction.

The form documents accountability and ensures destruction aligns with retention policies and any active legal or regulatory holds.

Step-by-step: Completing the request

A clear sequence ensures timely review, approval, and compliant destruction.

  • 01
Prepare details: Gather record identifiers, date ranges, and custody information.
  • 02
Complete form: Enter requested fields and attach inventories where needed.
  • 03
Obtain approvals: Route to custodian and compliance or legal for sign-off.
  • 04
Schedule destruction: Coordinate with provider and document the disposal date.

Where to submit and how routing works

Understand routing so the request reaches legal, compliance, or the designated records manager without delay.

  • Internal Records Team: Primary submission destination for verification and scheduling.
  • Compliance / Legal: Review for active holds, litigation risk, or regulatory constraints.
  • Third-party Vendor: Send certified lists and approvals when using external destruction services.
  • Final Archive: Confirm retention metadata update and archive the signed request.

Configuring an online request workflow

Recommended workflow settings reduce manual handoffs and capture required verification automatically.

Field Configuration
Authorization Required Two approver workflow, custodian plus compliance
Signer Authentication Email plus SMS code for approvers
Attachments Allow inventory upload in PDF or CSV
Audit Capture Enable timestamp, IP, and action log

Technical delivery and format considerations

Select platforms and file formats that preserve the signed record and audit trail.

  • Accepted formats: PDF, DOCX, and scanned TIFF
  • Integrations: Supports Salesforce, NetSuite, Google Workspace
  • Authentication: Email, SMS code, or stronger MFA

Store the final signed form and its audit certificate in records systems or secure cloud storage for retention and eDiscovery.

Essential sections every professional form should include

Design the form to capture authorization, inventory, method, and evidence to support compliance and audits.

Request Details

Include requester name, contact, department, and request date so the origin of the instruction is auditable and verifiable during reviews.

Record Inventory

Provide a clear listing or attach a manifest with identifiers, date ranges, and counts for each record set slated for destruction.

Retention Confirmation

Document that applicable retention periods have lapsed and that no legal holds, tax requirements, or active investigations prevent destruction.

Destruction Method

Specify the secure disposal technique and vendor procedures, including certificates of destruction when a third party is used.

Approvals

Designate required approvers and capture signed authorizations with timestamps to show internal consent and oversight.

Certification

Provide a dated certification of completion after destruction, including vendor certificate numbers or internal confirmation.

Required identifying information on the form

Requestor: Full legal name
Custodian: Department and custodian name
Record ID: File identifiers or reference numbers
Date Range: MM/DD/YYYY format
Destruction Type: Shred/erase/incinerate
Approver: Signed approver name and date

Common mistakes that cause delays or compliance problems

  • Failing to check for active legal or litigation holds before destruction, resulting in spoliation risk and potential sanctions or reopening of matters.
  • Entering vague record descriptions without identifiers, making it impossible to verify whether retention requirements have been met.
  • Using unsecured methods for electronic media disposal without certified wiping or verification, risking data recovery and exposure.
  • Routing approvals to the wrong stakeholders or failing to capture a complete audit trail, which invalidates the destruction certificate.

Key legal and operational risks

Spoliation Risk: Court sanctions
Regulatory Fines: HIPAA or sector fines
Data Breach: Unauthorized disclosure
Contract Liability: Breach of contract claims
Tax Penalties: Loss of required records
