What the Document Missing Page Form Is

The Document Missing Page Form is a short administrative declaration used to record, explain, and replace one or more pages that were omitted from an otherwise complete document set. It documents which page(s) are missing, the circumstances of the omission, and any corrective steps taken, and it creates a clear audit trail for the file. Organizations use it to prevent processing delays, satisfy recordkeeping requirements, and provide a written basis for adding or notarizing replacement pages while preserving the integrity of the original document package.

Why documenting a missing page matters

A clear missing-page declaration reduces processing errors, supports compliance with record retention rules, and protects parties from disputes over incomplete records. It preserves auditability and helps determine whether re-execution, notarization, or supplemental filings are required.

Why documenting a missing page matters

Who typically completes this form

Parties that encounter incomplete paper or electronic packages usually complete the Document Missing Page Form to record the gap and the remedy.

  • Real estate closing agents and title officers who discover omitted disclosures or pages in deed packets.
  • Healthcare records staff documenting missing pages in patient charts or consent forms.
  • Legal support staff and paralegals correcting incomplete filing sets for court or client delivery.

The form helps both senders and recipients create a defensible record and triggers any follow-up required by policy or law.

Core elements of a professional missing-page declaration

A reliable form is concise, uses standardized fields, and creates an auditable link to the original document set so reviewers can confirm context and corrective action.

Document ID

Unique identifier or file number that links this form to the exact document set, ensuring reviewers can locate the original materials and confirm the missing pages.

Page(s) Missing

Specific page numbers and total pages originally expected; list each missing page to avoid ambiguity and to support any replacement sequencing or reassembly.

Reason for Omission

Concise explanation of how the page was omitted (scanning error, clerical removal, damaged page) so reviewers can assess risk and corrective needs.

Corrective Action

Describe actions taken (rescanned, reprinted, replacement attached, re-notarized) and include dates and responsible person to document chain of custody.

Signatory Attestation

Signature block for the person certifying the statement; include printed name, title, organization, and date to establish accountability.

Attachment List

Inventory of any replacement pages, exhibits, or supporting affidavits attached to the form so the full record is self-contained and auditable.

Step-by-step: completing the missing-page form

Follow these steps to record and remediate a missing page quickly and consistently.

  • 01
Identify: Confirm which page(s) are missing and why.
  • 02
Record: Enter details on the form using required formats.
  • 03
Attach: Add scanned replacements or supporting affidavits.
  • 04
Sign: Collect signature, date, and any required notarization.

How the form moves through your process

A clear workflow reduces rework; below are common routing steps to integrate into existing processing.

  • Report: Staff files the form and attaches replacement pages.
  • Review: Supervisor or compliance reviews and approves.
  • Finalize: Signatures and notarization, if required, are collected.
  • Archive: Store the completed packet with the original records.

Configuring a digital workflow for this form

Set up fields and routing rules so missing-page events are captured, assigned, and resolved automatically.

Field | Configuration Responsible | Value
Auto-attach scan Enable automatic upload from scanner to record
Reviewer assignment Route to compliance queue after submission
Signature requirement Require signer and date fields before close
Retention flag Mark record for extended retention if legal hold applies

Digital delivery and signing considerations

Ensure the platform you use supports secure attachments, audit trails, and required authentication methods.

  • File formats: Accept PDF and DOCX for scans and replacements
  • Audit trail: Capture IP, timestamp, and signer attribution
  • Authentication: Support email, SMS, or stronger KBA if required

Confirm the platform meets any regulatory or organizational controls before relying on electronic submission as the final record.

Timing and deadlines to keep in mind

Document timing can affect whether a replacement page can be accepted or whether re-execution is required; track discovery and remediation dates carefully.

Discovery date:

Date missing pages were identified; record as MM/DD/YYYY.

Remediation target:

Set an internal deadline to attach replacements and complete attestation.

Filing deadlines:

If original set was subject to a filing deadline, remediate before the regulatory due date.

Notarization window:

Allow time for scheduling notary or RON session if required.

Audit retention start:

Retention begins when the record is completed and finalized.

Key milestones from discovery to archive

Use a sequential checklist to ensure each stage is completed and documented before moving to the next.

01

Record Discovery

Log the omission and capture initial details and date.

02

Obtain Replacement

Rescan or retrieve the missing page and verify content.

03

Attest and Sign

Signer certifies the replacement and signs the form.

04

Archive and Notify

Attach to original file and notify stakeholders of completion.

Common errors to avoid

  • Failing to tie the form to a unique document ID, which can lead to misplaced replacements and unresolved discrepancies.
  • Using ambiguous language about which pages are missing instead of listing exact page numbers and locations in the packet.
  • Skipping required attestations or failing to collect required notarizations, causing regulatory or filing rejection.
  • Not recording dates in a standardized format, which complicates timelines and can void remedies tied to statutory deadlines.

Risks and potential consequences of incomplete handling

Processing Delay: May cause multi-day to multi-week delays
Filing Rejection: Replacement may be refused without correct attestation
Regulatory Noncompliance: Can trigger audits or administrative penalties
Contractual Disputes: Parties may dispute authenticity or completeness
Data Integrity Risk: Untracked replacements weaken chain of custody
Financial Exposure: Potential costs for re-filing or legal review
