What the Donation Acknowledgment Release Form Is

A Donation Acknowledgment Release Form documents a donor's contribution and confirms the charitable organization’s receipt while releasing the charity from future claims tied to that specific donation. The form typically records donor identity, donation description, fair market valuation or estimate, the date of transfer, any restrictions or conditions, and a signature block where the donor and an authorized representative acknowledge terms. Donors rely on this document to substantiate tax deductions when allowed by IRS rules and to clarify ownership and use of contributed property.

Why this form matters for donors and charities

A clear Donation Acknowledgment Release Form protects donor tax substantiation, records transfer of title, and limits future disputes by documenting consent and any use restrictions; it supports compliance with IRS requirements for charitable contributions and internal recordkeeping.

Why this form matters for donors and charities

Who commonly completes this form

Organizations and individuals involved in donations use this form to document transfers, ensure tax compliance, and record any release or conditions agreed by both parties.
