What the Donation Charity Form Is and when it’s used

A Donation Charity Form is a written acknowledgment used by nonprofit organizations to document gifts from donors, record donor contact and payment details, specify any restrictions on use, and provide the supporting information donors need for tax substantiation. The form commonly captures donor name, mailing address, donation amount or description of non‑cash property, date of contribution, whether goods or services were provided in return, and a space for the authorized nonprofit signature. Organizations use it to create consistent receipts for donors and to support IRS reporting and donor tax deduction claims.

Why a clear Donation Charity Form matters

A well‑crafted Donation Charity Form protects donors and nonprofits by documenting the transfer, clarifying restrictions, and enabling substantiation for tax deductions under federal rules. It reduces disputes, supports internal controls and audit trails, and helps charities comply with IRS substantiation requirements and state recordkeeping standards.

Why a clear Donation Charity Form matters

Who completes and signs this form

Roles should be clearly identified on the form to ensure proper authorization, to allocate restricted funds correctly, and to maintain an auditable record for donors and regulators.

  • Individual donors who need a written receipt for income tax deductions or personal records
  • Charity development or finance staff responsible for donor records, acknowledgments, and reporting
  • Third‑party processors or volunteer fundraisers submitting consolidated donation information

Essential sections to include on the Donation Charity Form

A professional form organizes donor and gift details, substantiation language, signature blocks, and internal tracking fields. Clear field labels and conditional fields for in‑kind gifts or pledges make the form useful for audits, donor communications, and downstream reporting.

Donor information

Full legal name, mailing address, email, and taxpayer identification when required for recordkeeping and donor follow‑up.

Gift description

Monetary amount or detailed description of non‑cash items, including fair market value estimates and quantity when applicable.

Date and method

Contribution date and payment method (check, ACH, credit card, in‑kind) to establish timing for tax purposes.

Restricted purpose

Clear designation if the donor restricts the gift to a program, project, or capital fund; include permissions for redirection if allowed.

Acknowledgment language

Statement describing whether goods or services were provided in exchange and any estimate of their value for IRC compliance.

Internal tracking

Fund or program code, donor ID, and staff initials to support CRM updates and audit trails.

  • Integrations: Connect to CRM, payment processor, and cloud storage

Timing considerations and critical deadlines

Timing affects tax substantiation, recognition periods, and reporting; track when the donation was made and when the acknowledgment was issued.

Gift date importance:

Date of contribution determines tax year for donor deductions

Acknowledgment for $250+:

Issue written acknowledgement per IRC §170(f)(8) for single contributions of $250 or more

Recurring gifts:

Provide annual summary for recurring donations to aid donor tax filings

Non‑cash appraisals:

Obtain qualified appraisal when required by IRS rules before filing

Record retention:

Retain records according to federal and state retention schedules

Key processing milestones for each donation

Track milestones from receipt to final accounting to ensure compliance and timely donor acknowledgement.

Common preparation pitfalls to avoid

  • Incomplete donor contact details that prevent follow‑up and tax substantiation
  • Failing to state whether donor received goods or services in return, which affects deductible amount
  • Using vague descriptions for non‑cash gifts without fair market value or appraisal notes
  • Not mapping restricted gifts to internal fund codes, causing misallocation in financial reports

Risks and potential penalties for errors

IRS substantiation: Failure to provide required written acknowledgment for $250+ gifts can disallow donor deductions per IRC §170(f)(8)
I‑9/Employment risk: N/A — not applicable to donation receipts
Misstated value: Overstating non‑cash value risks donor disallowance and reputational harm
Privacy breach: Improper handling of donor PI may trigger state data breach laws and obligations under CCPA
Recordkeeping: Insufficient records can complicate audits and grant reporting
Public trust: Errors undermine donor confidence and can reduce future fundraising effectiveness

Data elements to protect and why

Donor PII: Name, address, email, phone
Payment data: Card or bank details—treat as PCI data
Gift value: Monetary amounts and valuations
Health info: Protected health information if present
Restricted terms: Designation and usage restrictions
Audit trail: Signatures, timestamps, IP/log data

Supporting documents commonly attached

Include documents that substantiate the gift, clarify donor intent, or support tax deductions; these become part of the permanent file.

Gift agreements

Detailed agreements for restricted or large gifts describing terms, reporting obligations, and effective dates.

Appraisals

Qualified appraisal for non‑cash gifts when required under IRS rules; attach appraisal summary and appraiser contact.

Payment receipts

Bank deposit slips, credit card transaction records, or payment processor confirmations supporting the recorded amount.

Board approvals

When acceptance of a restricted or unusual gift requires board approval, include resolution or minutes.
