What the Donation Form for Faith Based Giving Is

The Donation Form for Faith Based Giving is a written record used by congregations, religious nonprofits, and faith-based programs to capture donor intent, identify the donor, document the amount and designation of the gift, and provide the acknowledgment needed for tax substantiation. It typically records donor contact and tax identification information, payment method, whether the gift is restricted or unrestricted, and the date. The form also includes the organization’s verification and signature blocks to confirm receipt and to support accounting, stewardship, and audit requirements.

Why a Clear Donation Form Matters

A well-structured donation form protects donors and the organization by documenting intent, enabling accurate receipting for IRS purposes, and supporting internal controls for funds designated to specific ministries or programs.

Why a Clear Donation Form Matters

Who Typically Uses This Donation Form

This form is used by faith organizations and their donors to record gifts and produce tax acknowledgments.

  • Congregation finance teams and treasurers who process gifts, reconcile accounts, and prepare donor acknowledgments for tax purposes.
  • Development staff and gift officers who track restricted funds, pledge payments, and stewardship communications for ministries and campaigns.
  • Individual donors and family representatives who need a written receipt for income tax deduction substantiation and recordkeeping.

Proper use reduces accounting errors, ensures compliance with charitable accounting best practices, and improves transparency between donors and the organization.

  • Omitting payment transaction details for electronic gifts, which hinders reconciliation and increases the risk of untracked funds.
  • Not providing a contemporaneous written acknowledgment for gifts $250 or more, jeopardizing the donor’s ability to claim a tax deduction.
