What a Donation Give Through document is and where it fits

A Donation Give Through is a written authorization that lets a donor direct a gift to a beneficiary through an intermediary or program (for example, a payroll giving program, donor-advised fund, or workplace charitable portal). It captures donor identity, recipient details, donation amount or in-kind description, and an explicit authorization to transfer funds or assets. The form is used to document donor intent, support tax substantiation, and create an audit trail for nonprofit and payroll administrators; it can be executed on paper or electronically under U.S. e-signature law.

Why using a Donation Give Through form matters

A clear, complete form documents donor intent, reduces processing errors, enables consistent tax receipts, and creates an auditable record for the donor, intermediary, and recipient while supporting regulatory and nonprofit accounting requirements.

Why using a Donation Give Through form matters

Typical users and roles for this document

Each role has different responsibilities: donors must provide accurate identity and intent, processors must verify recipient details, and administrators must retain records for compliance and audit.

  • Individual donors submitting gifts through an employer, donor-advised fund, or third-party platform to an intended beneficiary.
  • Nonprofit gift-processing or development staff who verify donor intent and issue tax substantiation.
  • Payroll, HR, or corporate giving coordinators who process matched gifts and routing to recipient organizations.

Core elements of a professional Donation Give Through form

A well-structured form contains discrete sections that make verification, routing, and tax reporting straightforward for all parties.

Donor Information

Full legal name, contact details, and taxpayer identification where applicable; this enables identity verification and tax substantiation for the donor and recipient.

Recipient Details

Organization name, mailing address, and Employer Identification Number (EIN) or fiscal agent details so funds are routed accurately and tax receipts reference the correct beneficiary.

Donation Description

Specify amount, currency, or detailed description of in-kind gift, including any restrictions or designations that affect acceptance or use by the recipient organization.

Authorization Statement

Clear language by which the donor authorizes the intermediary to transfer or allocate the gift, including any matching or timing instructions that govern processing.

Tax Acknowledgment

A recipient-confirmation block for filing the acknowledgement required under IRS rules; include language on deductible status and any goods or services exchanged.

Signature & Audit Trail

Signed and dated signature block plus a record of signer identity, timestamps, and processing steps needed to support electronic execution and later verification.

Essential data fields to collect

Donor name: Full legal name
Donor address: Street, city, state, ZIP
Recipient EIN: Tax ID number
Donation amount: Dollar amount or description
Effective date: MM/DD/YYYY
Signature: Signer name and date

Step-by-step: completing and submitting a Donation Give Through

Follow these steps to prepare, authorize, and route a donation through an intermediary or program.

  • 01
Gather documents: Collect donor ID, EIN, and any prior authorization
  • 02
Enter donor and recipient: Complete the form fields accurately
  • 03
Verify details: Confirm EIN and recipient acceptance
  • 04
Sign and submit: Provide signature and route to administrator

Configuring an online Donation Give Through workflow

Typical configuration settings for an electronic template and routing workflow used by administrators and payroll/DAF providers.

Field Mapping Map donor/recipient fields to CRM or donor database
Authentication Email link or SMS code; stronger KBA optional
Template Name Use a consistent name for auditing
Notification Rules Email confirmations to donor and recipient
Retention Setting Store signed PDF and audit trail per policy

Where to send or file the completed form

After execution, route the signed form to the party responsible for remittance, recordkeeping, and tax acknowledgment.

  • Recipient organization: Send a copy for donation acceptance and receipt
  • Intermediary or DAF: Forward instructions and authorization for distribution
  • Payroll/HR: Submit cutoff-sensitive donations via payroll channels
  • Internal records: Retain a signed copy for compliance and audit

Technical and platform considerations for e-completion

Ensure the selected solution preserves an auditable certificate of completion and meets applicable compliance needs (for example HIPAA BAA when health data is present) before accepting electronic signatures.

  • Document formats: PDF, DOCX, or HTML accepted
  • Authentication: Email link, SMS code, or stronger options
  • Integrations: CRM, payroll, or donor database integrations

Key timing considerations and common deadlines

Timing affects tax-year treatment, payroll cycles, and intermediary cutoff dates; verify dates with the recipient or program administrator.

Tax-year deadline:

Gift must be completed by Dec 31 for that tax year

Payroll cutoff:

Employer match often requires submission before payroll processing

DAF acceptance:

DAF providers may have monthly or quarterly processing windows

Notarization timing:

If notarization is required, sign in the notary's presence

Acknowledgement issuance:

Recipient issues receipt after funds or assets are received

Common mistakes to avoid when preparing this form

  • Entering an incorrect EIN for the recipient, which can prevent issuance of a valid tax acknowledgement and delay processing.
  • Mismatched donor name or TIN that does not match IRS records, potentially triggering backup withholding or requiring corrected receipts.
  • Omitting an explicit authorization or designation, causing ambiguity about donor intent and how funds should be used.
  • Failing to observe payroll or intermediary cutoffs, which can defer the donation into the next tax year and affect deductibility.

Potential consequences of errors

Incorrect TIN: Backup withholding 24%
Late processing: Gift applies to next tax year
Missing authorization: Donation may be refused
Tax deduction risk: Insufficient substantiation
Recordkeeping lapse: Audit exposure
Intent disputes: Legal or administrative review

Practical examples of common Donation Give Through scenarios

Two concise scenarios illustrate how the form supports different channels and processing paths.

Case Study 1

A donor authorizes payroll deduction to a local food bank

  • deduction begins next pay period
  • the employer forwards funds monthly and the food bank issues a consolidated annual tax acknowledgement to the donor.

Case Study 2

An individual directs an in-kind gift of equipment through a donor-advised fund

  • DAF accepts conditionally pending appraisal
  • the DAF coordinates transfer, appraisal, and provides the donor with a charitable contribution acknowledgment.

Practical tips for accurate and efficient completion

Follow these practices to reduce errors, speed processing, and protect donor and recipient interests.

Validate recipient EIN
Confirm the recipient’s EIN against IRS or official records before submission to ensure the tax acknowledgement references the correct legal entity and avoids misdirected funds.
Record donor consent
Capture explicit donor consent for electronic records and retain the consent evidence alongside the signed form to meet ESIGN and IRS substantiation requirements.
Respect timing rules
Submit gifts before payroll or intermediary cutoffs and record the effective date to ensure the contribution is applied to the intended tax year.
Keep complete audit trails
Store the signed document, timestamps, signer authentication events, and processing notes to support reconciliation and any future audit inquiries.
