What the Door Relocation Request Form Is and When It’s Used

The Door Relocation Request Form is a standardized document used to request moving or modifying an interior or exterior door within a building or property. It captures project identifiers, current and proposed door locations, dimensions, material and hardware specifications, accessibility and fire-safety considerations, attachments such as drawings or photos, and required approvals from facilities, code compliance, or property management. Organizations use this form to create a clear record for permits, budget estimates, contractor work orders, and sign-off workflows that document who approved the change and when.

Why a Formal Request Form Matters

A structured Door Relocation Request Form reduces miscommunication, documents regulatory and safety checks, and creates an auditable approval trail. It streamlines permitting, reduces rework, and helps facilities teams verify accessibility, fire code, and warranty implications before work begins.

Why a Formal Request Form Matters

Who typically completes and reviews this form

Several roles create, approve, or act on a door relocation request depending on project scope and property type.

  • Building or property managers who coordinate approvals, budget, and contractor scheduling for on-site work.
  • Architects, engineers, and designers who confirm structural, accessibility, and code compliance before relocation.
  • General contractors or maintenance supervisors who use the form to scope work and prepare cost estimates.

Clear role delineation on the form speeds approval, reduces liability, and ensures work meets local codes and owner requirements.

  • The record streamlines internal approvals, creates a contractor scope, and archives the signed approval for warranty and lease compliance.

Small Contractor Example

A contractor receives a relocation request from a facilities director

  • The request includes as-built photos and hardware specs
  • The contractor uses the form to price the job, schedule labor, and deliver a signed completion record to the owner.
