The form captures contact details, driver license information and class, endorsements, vehicle assignments, training records, citation and accident history, insurance declarations, and consent for background checks. Driving History

Provide a five-year summary of accidents, moving violations, suspensions, or revocations. Include dates, docket numbers when available, and any remediation or training completed to establish mitigation efforts for risk assessment.

Vehicle Assignment

List primary vehicle(s) assigned, VINs, permitted vehicle classes, and mileage limits. Note personal versus company-owned vehicles, and include restrictions tied to license class or insurance coverage.

Insurance

Document current policy numbers, carrier names, coverage limits, and effective dates. Attach declarations pages to verify liability limits and ensure drivers meet minimums required for assignment and company insurance policies.

Consent & Authorizations

Include explicit dated consent for criminal background checks, motor vehicle record (MVR) pulls, and periodic rechecks. State retention and withdrawal procedures to comply with ESIGN consumer-disclosure expectations when required.

  • Leaving checkbox consent blank for background checks leads to incomplete screening and delays in hiring, assignment, or insurance verification processes.
  • Collecting free-text responses without standardized fields increases transcription errors, prevents reliable automated checks against DMV records, and slows high-volume processing.
