What the EACCME Form Is and when it's used

The EACCME Form is a standardized accreditation submission used by continuing medical education (CME) providers to request credit recognition from the European Accreditation Council for Continuing Medical Education. It collects structured data about the activity, learning objectives, faculty disclosures, educational format, duration, and assessment methods. While the form is centered on European accreditation requirements, many U.S.-based providers preparing transnational CME or joint-accredited activities use the same template to document learning outcomes, conflicts of interest, and evaluation plans before submission to a national or international accreditor.

Why a clear EACCME Form matters for providers

A complete, well-structured EACCME Form speeds accreditation review, reduces follow-up questions, and improves the likelihood of timely credit assignment.

Why a clear EACCME Form matters for providers

Who commonly prepares and signs the EACCME Form

Typical users include CME directors, medical education coordinators, and designated accreditation contacts at sponsoring organizations.

  • CME Director or Medical Education Lead responsible for content, objectives, and compliance with accreditation standards.
  • Accreditation Administrator who compiles disclosures, faculty credentials, and supporting evidence for the submission package.
  • Finance or Operations staff who confirm fees, invoicing details, and institutional approvals for the application.

Smaller providers and independent course organizers may use the same workflow but often assign a single staff member to act as submitter and record keeper.

Stepwise process to complete and submit the EACCME Form

Follow these sequential actions to prepare a submission that aligns with accreditation expectations.

  • 01
Assemble materials: Collect agenda, objectives, CVs, and disclosures before starting the form.
  • 02
Complete fields: Enter data carefully and use MM/DD/YYYY for dates.
  • 03
Attach documents: Upload PDFs for agendas and slides; verify file names.
  • 04
Review and submit: Peer-review the record and submit via the accreditor's portal or chosen delivery method.

How to set up an online EACCME submission workflow

Map digital settings to your internal review and approval steps so submissions remain consistent and auditable.

Field mapping Match each form field to your internal metadata and reporting columns.
Approval routing Require sequential approvals: content lead, compliance reviewer, and signatory.
Notifications Enable email alerts for pending approvals and final submission confirmations.
Conditional fields Show or hide fields for virtual vs in-person activities to reduce clutter.
Retention policy Configure automatic archival after the event plus the required retention period.

Where the completed form should be sent

Understand final destinations and typical routing to ensure timely processing and proper record storage.

  • Accreditor portal: Primary destination for formal EACCME submissions and reviewer access.
  • Institutional archive: Store a copy in your organization’s records management system for audit purposes.
  • Faculty records: Provide signed confirmation copies to lead faculty and moderators.
  • Learner certificates: Ensure accreditation details feed into certificate issuance systems.

Technical considerations for digital completion and signing

Choose a platform that supports secure uploads, role-based access, and a reliable audit trail for each signed submission.

  • File formats: PDF and DOCX supported
  • Authentication: Email or multi-factor available
  • Integrations: Works with LMS and document repositories

Essential features of a professional EACCME Form package

A complete submission combines structured form fields with supporting evidence and clear disclosures to meet accreditation standards.

Structured metadata

Consistent fields for provider name, activity title, dates, and credit hours enable accurate indexing and reduce reviewer queries during accreditation assessment.

Conflict disclosures

Consolidated faculty disclosure statements with mitigation plans demonstrate transparency and help reviewers evaluate potential bias risks in educational content.

Assessment plan

Clear description of learner assessment methods, evaluation instruments, and sample questions supports the accreditor’s review of educational outcomes.

Audit trail

Signed submissions should include timestamps, signer identity data, and an immutable activity log to support compliance and future audits.

Key components to include on every EACCME Form

Ensuring each component is complete reduces the chance of conditional approval and speeds final accreditation decisions.

Header

Provider identification, official contact details, and sponsoring institution clearly listed to enable accreditation contact and verification.

Activity details

Exact activity title, format (live, online), total duration, and proposed credit hours for learner recognition.

Learning objectives

Concise, measurable objectives describing expected learner outcomes and aligned to educational activities and assessment.

Faculty disclosures

Full financial and non-financial disclosure statements for all presenters, with mitigation notes where conflicts exist.

Evaluation plan

Methods for measuring learner knowledge change or competence, sample evaluation items, and planned use of results.

Signatures

Authorized signatures from the provider representative, dates, and role descriptions demonstrating authority to submit.

Typical timing and deadlines for accreditation submissions

Deadlines vary by accreditor and activity type; confirm the accreditor’s published submission windows and lead times early.

Submission lead time:

Submit 8–12 weeks before the scheduled activity for full review and prior approval.

Expedited requests:

Some accreditors allow expedited review with shorter timelines and additional fees.

Evaluation reporting:

Post-activity evaluation reports often due 30–90 days after the event conclusion.

Certificate issuance:

Accredited credit statements should be available to learners within 30 days of final approval.

Appeals and corrections:

Allow 14–30 days for appeals or submission corrections after initial reviewer feedback.

Key submission milestones from draft to accreditation decision

Track these sequential milestones to keep stakeholders informed and avoid last-minute delays.

01

Draft preparation

Compile materials, write objectives, and confirm faculty disclosures before routing for internal review.

02

Internal approval

Obtain signatory approvals from content lead and compliance or legal as required by your organization.

03

Accreditor review

Submit the packet to the accreditor and await formal review or conditional requests for information.

04

Final decision

Receive accreditation decision and update learner materials and certificate language accordingly.

Common errors that delay EACCME Form approvals

  • Incomplete disclosures: Missing or inconsistent faculty financial disclosures trigger reviewer follow-up and can delay approval for weeks.
  • Unclear objectives: Vague learning objectives make it hard for reviewers to assess educational impact and may require rewriting.
  • Incorrect dates or durations: Misstated start/end times or total hours can change credit determinations and require resubmission.
  • Missing supporting materials: Lack of agenda, slide samples, or evaluation tools often leads to conditional approval requests or rejection.

Consequences of submitting incorrect or incomplete forms

Accreditation denial: Activity not accredited
Delayed credit: Learner certificates withheld
Financial impact: Refunds or fees possible
Reputational risk: Trust and participation affected
Audit exposure: Increased scrutiny in audits
Data risk: Noncompliant recordkeeping
