WHEREAS, the Student named above desires to enroll in the program identified below and agrees to pay the tuition and observe Institution policies as set forth in this Agreement;

WHEREAS, the parties intend by this document to set forth the terms of enrollment, payment, waiver of liability to the extent permitted by law, confidentiality of certain materials, and related administrative provisions.

Confidentiality and Materials

Institution materials, curricula, evaluations, and any proprietary tools provided to the Student remain confidential. Student shall not reproduce or distribute proprietary materials without prior written permission. Institution agrees to protect Student personal data in accordance with applicable privacy laws and only disclose records as required by law or with the Student's written consent.

Waiver, Assumption of Risk, and Media Release

The Student, or the Parent/Guardian if Student is a minor, acknowledges that participation in educational activities may involve inherent risks including but not limited to physical injury during practical exercises or off-site activities. To the fullest extent permitted by law, the Student releases, waives, and forever discharges the Institution, its officers, employees, instructors, and agents from any and all liability, claims, demands, actions, or causes of action for injury, loss, or damage arising out of or related to participation, except where caused by Institution's gross negligence or willful misconduct.

Media Release: Student grants Institution permission to photograph or record the Student during program activities for non-commercial educational and promotional use unless the box below is checked to withhold consent.

By signing below, the signer confirms they have read, understand, and agree to the terms and waivers contained in this Agreement and represent they have authority to bind the Student if signing as Parent/Guardian.

Refund and Cancellation Policy

Refunds for withdrawal or cancellation are provided according to the schedule set by the Institution. Unless otherwise provided in writing, administrative fees may be retained.
