Authorization, Terms and Notices

By signing below, Cardholder/Account Holder authorizes Merchant to initiate debits to the nominated card or bank account for the amounts and schedule set forth in this form. This authorization permits Merchant to process one or more entries to the Customer's payment method as required to effect the transaction(s) described herein.

Customer acknowledges and agrees that: Merchant may store payment credentials in a secure tokenised format; Customer will notify Merchant of any changes to account or card details; Merchant may impose a fee for declined payments or returned items; Merchant will provide a receipt or statement for each processed payment upon request.

Cancellation: Customer may revoke this authorization by providing written notice to Merchant at least days prior to the next scheduled debit. Revocation does not affect debits already processed in accordance with this authorization.

Changes to Payment Amount: For recurring amounts, Merchant will provide notice of any change in amount at least days prior to effect. If no notice period is required by law, Merchant will supply reasonable advance notice where practicable.

Liability and Disputes: Customer shall notify Merchant promptly of any disputed transaction. Merchant's liability for errors or unauthorized debits is limited to correcting the error and, where applicable, refunding the amount in accordance with Merchant policies and applicable law.

Privacy and Security: Customer consents to Merchant collecting and storing personal and payment information necessary to process payments. Merchant will handle such information in accordance with its privacy obligations and will not disclose payment information except as required to process transactions, to its service providers, or as required by law.
