What the Election Board Nomination Form Is and when it matters

An Election Board Nomination Form is the formal written statement a prospective candidate submits to a local or state election authority to appear on a ballot. It typically identifies the candidate, the office sought, party affiliation where applicable, nominating signatures or endorsements, and required attestations. States set eligibility, signature thresholds, notarization, and filing windows; county election officials then review and accept or reject filings. The form starts the official nomination process and begins administrative checks for eligibility, residency, and signature validity.

Why a clear, correct nomination form matters

A properly completed Election Board Nomination Form ensures a candidate’s eligibility is verified, prevents avoidable rejections, and preserves ballot access rights. Accurate submission reduces administrative delays, minimizes legal challenges, and creates a clear audit trail for election officials.

Why a clear, correct nomination form matters

Who prepares and signs these nomination forms

Clear role separation and a checklist reduce errors, speed processing, and help ensure filings comply with state statutes and local rules.

  • Campaign staff or volunteers who collect signatures, assemble supporting documents, and verify completion before submission to the election authority.
  • Candidates who must provide personal details, attestations, and the final signature to confirm eligibility and intent to run.
  • County or state election clerks who accept, review, notarize (if required), and certify nominations following statutory procedures.

Core components to include on a professional nomination form

A professional Election Board Nomination Form groups candidate identification, office and district details, nominating endorsements or petition data, signature blocks, attestations, and a filing certification section for election officials.

Candidate Name

Full legal name as shown on government ID; inclusion of suffixes or middle names avoids mismatches during verification processes.

Office & District

Exact office title and district or ward number. Precision determines ballot placement and verifies geographic eligibility.

Party Affiliation

If required, state party selection and any primary designation; mismatched party information can disrupt nomination eligibility.

Nominating Signatures

Count of required signatures, petition attachment point, and space for each signer’s printed name, address, and signature.

Attestation

Candidate declaration of eligibility, residency, and compliance with nomination laws, often signed under penalty of perjury.

Filing Certification

Reserved for election official: date/time received, index number, and accept/reject checkbox with brief reason field.

Step-by-step: complete and submit a nomination form

Follow these essential steps in order to prepare a valid nomination package and meet filing requirements.

  • 01
Prepare Document: Download the official form from the election office and review instructions.
  • 02
Collect Signatures: Gather required nominator signatures using the correct petition format.
  • 03
Verify Identity: Confirm candidate and signer IDs match written names and addresses.
  • 04
File With Office: Submit the completed form and attachments to the designated filing office within the deadline.

How to configure an online nomination workflow

Set up a consistent digital workflow so forms are filled, signed, and routed securely to the election office.

Field Configuration
Template Lock form fields and add required signatures
Authentication Use email plus SMS code for signer verification
Notifications Auto-notify staff when signers complete fields
Archive Format Save final PDF/A with audit trail

Where completed nomination forms typically go

After completion, follow your jurisdiction’s routing rules to deliver the package to the correct official or office.

  • County Election Office: Primary recipient for local race filings
  • State Secretary of State: Receives statewide office nominations
  • Party Office: Some parties require parallel filing for primary access
  • Campaign Records: Retain a certified copy in campaign archives

Digital delivery and eSubmission considerations

Use a secure platform that supports required file formats, signer authentication, and audit trails when submitting or archiving nomination forms.

  • File Formats: PDF, PDF/A supported
  • Integrations: Works with Google Workspace
  • Authentication: Email + SMS or KBA

Timing and deadlines to watch before filing

Nomination filing windows, signature collection periods, and certification deadlines vary by state and office; confirm exact dates with the appropriate election official well ahead of filing.

Signature Collection Period:

Start and end dates depend on state statute

Filing Deadline:

Often several weeks before primary or general election

Certification Date:

Election authority sets official certification timeline

Ballot Access Cutoff:

Final administrative checks conclude before ballot printing

Withdrawal Window:

Statutory window to withdraw nomination in some states

Key milestones from draft to certified filing

Track these sequential milestones to keep the nomination on schedule and documented from preparation through certification.

01

Draft and Review

Prepare form and check for missing fields and ID mismatches.

02

Signature Gathering

Collect and verify nominator signatures per formatting rules.

03

Notarization / Attestation

Complete any required notarizations or witness attestations.

04

File and Certify

Submit to election office and obtain receive-stamp or certification.

Common preparation errors that lead to rejection

  • Missing or inconsistent candidate name formats between ID and form cause verification failures and administrative hold-ups.
  • Insufficient or improperly formatted nominator signatures — address or signer data mismatches frequently trigger rejection.
  • Failing to notarize when state law requires it, or submitting only an electronic copy when a wet signature is mandated.
  • Late filing or submitting to the wrong office — jurisdictional misrouting often nullifies otherwise valid nominations.

Consequences of incorrect or late nomination filings

Disqualification: Late or noncompliant filings may be rejected
Legal Challenge: Opponents can contest candidacy
Campaign Delay: Ballot access may be delayed or denied
Fines: Some jurisdictions impose administrative fines
Reputational Risk: Public disputes can damage campaign credibility
Recordkeeping Liability: Failure to retain required records can cause compliance issues

Essential information fields every nomination form should capture

Candidate Identity: Full legal name
Office Details: Office and district
Residence: Street address
Nominator Data: Printed name and address
Signature Block: Candidate signature and date
Filing Stamp: Office acceptance record

Practical scenarios showing typical nomination workflows

Two representative scenarios illustrate how a nomination form moves from campaign collection to official filing and certification.

Local Candidate Case

A volunteer team collects 150 petition signatures following the county format.

  • Signatures are verified for address and date.
  • The campaign submits a completed packet to the county election office, retains certified copies, and archives scanned PDFs with signed audit trails for contest readiness.

County Clerk Case

A county clerk receives mixed paper and electronic submissions during a filing window.

  • Staff validate required fields and notarizations.
  • Rejected items are logged with reasons, and clerks issue detailed cure notices so candidates may correct deficiencies before certification deadlines.
