Scope of Notification

The Organization will use the contact information provided to notify the named emergency contacts if the Individual is injured, becomes incapacitated, or is otherwise in need of immediate assistance. The Organization's actions may include contacting emergency medical services, notifying next of kin, arranging transport, and communicating with medical providers to the extent authorized below.

Authorizations and Consents

By checking the boxes below, the Individual grants the Organization the limited authorizations described. Each consent is severable; refusal of one does not affect others.

Confidentiality

The Organization will treat the Individual's emergency contact and medical information as confidential and will not disclose it except to the extent necessary to perform emergency notification and response, to obtain medical treatment, to comply with legal obligations, or as authorized in writing by the Individual.

Governing Law

This Emergency Contact Document is governed by and construed in accordance with the laws of the state specified below without regard to conflict-of-law principles.

Entire Agreement

This document, together with any attachments or appendices executed by the parties, constitutes the entire agreement between the parties with respect to emergency contacts and supersedes all prior or contemporaneous oral or written agreements on that subject.

Certification

The Individual certifies that the information provided is true and accurate to the best of their knowledge and that they have authority to provide the listed emergency contact details. The Individual further acknowledges understanding the authorizations granted herein and may revoke such authorizations in writing subject to the notice provisions above.
