WHEREAS, the parties desire to set forth the scope of use, confidentiality safeguards, duration, and related administrative terms governing such emergency contact information.

Scope of Work

Organization will collect, store, and use the emergency contact information provided by Individual solely for the purposes of emergency notification, medical response coordination, and essential administrative communications. Organization will not use the information for recruitment, marketing, or unrelated distribution.

Payment Terms

The parties acknowledge that this Agreement primarily governs information handling; nevertheless, should Organization charge administrative fees for processing or verifying emergency contact data, the following payment terms shall apply.

Term and Termination

This Agreement is effective as of the Effective Date below and shall continue in effect until the Termination Date below unless earlier terminated in accordance with this section.

Confidentiality

Organization acknowledges that the emergency contact and medical information supplied by Individual constitutes sensitive personal information. Organization agrees to restrict access to such information to authorized personnel on a need-to-know basis, to store it using reasonable administrative and technical safeguards, and to not disclose it to third parties except as required by law, emergency responders, or with Individual's prior written consent.

Consent and Release

Individual authorizes Organization and its authorized agents to contact the listed emergency contacts in the event of an emergency involving Individual. Individual further authorizes Organization to disclose necessary medical and identifying information to emergency personnel and treating medical facilities. Individual releases Organization from liability for any action taken in reasonable reliance on the provided emergency contact information and this authorization.

Governing Law

This Agreement shall be governed by and construed in accordance with the substantive laws of the state or jurisdiction specified by Organization below without regard to conflict of law principles.

Entire Agreement

This Agreement, together with any attachments or written amendments signed by both parties, constitutes the entire agreement between Organization and Individual with respect to the subject matter hereof and supersedes all prior negotiations, understandings, and agreements, whether written or oral.
