EMPLOYEE FOSTER HANDBOOK AND EMPLOYMENT AGREEMENT

WHEREAS

WHEREAS, Organization Name: is duly authorized to operate foster care and placement services and employs staff to deliver care, supervision, and associated administrative duties; and

WHEREAS, Employee Name: seeks to provide foster care services and accepts employment subject to the policies, procedures, and obligations set forth in this Handbook and Agreement; and

WHEREAS both parties intend this document to define the scope of responsibilities, compensation, confidentiality, and termination procedures applicable to Employee's duties as foster personnel.

SCOPE OF WORK

Employee shall perform duties consistent with foster care placement, supervision, and related administrative requirements. Specific duties, performance standards and deliverables are described below and may be modified by Organization in writing as needed to meet regulatory and programmatic requirements.

PAYMENT TERMS

Compensation for services shall be as stated below. Organization shall pay Employee in accordance with the payment schedule and shall make applicable tax withholdings and deductions as required by law. Any reimbursements for approved expenses shall be submitted with receipts and are subject to Organization's reimbursement policy.

TERM AND TERMINATION

This Agreement commences on the Start Date below and continues until the End Date unless earlier terminated in accordance with this section. Employment is subject to applicable statutes and Organization policies.

Organization may terminate Employee immediately for cause, including but not limited to violations of child safety policies, falsification of records, criminal conduct, or gross negligence. Employee may be terminated without cause upon provision of the Notice Period set forth above or payment in lieu thereof as permitted by law.

CONFIDENTIALITY

Employee shall maintain in strict confidence all protected health information, personal data, proprietary program materials, placement records, and any other confidential information learned in the course of employment. Confidential information shall not be disclosed except as required by law or authorized in writing by Organization. This obligation survives termination for a period of five (5) years or as otherwise required by law.

Employee hereby acknowledges and agrees to comply with the confidentiality obligations of this Agreement.

POLICIES, PROCEDURES AND TRAINING

Employee shall comply with Organization policies regarding child protection, incident reporting, medication administration, background checks, and mandatory training required for foster personnel. Failure to complete required training or to comply with these policies constitutes grounds for corrective action up to and including termination.

GOVERNING LAW

This Agreement shall be governed by and construed in accordance with the laws of the State of without regard to conflict of law principles.

ENTIRE AGREEMENT

This document, together with any written appendices or attachments expressly incorporated herein, constitutes the entire agreement between Organization and Employee with respect to the subject matter herein and supersedes all prior negotiations, understandings, and agreements, whether written or oral. Any amendment must be in writing and signed by both parties.

ACKNOWLEDGMENT OF RECEIPT

Employee acknowledges receipt of this Handbook and Agreement, understands the policies and obligations contained herein, and agrees to abide by all applicable policies and lawful directives of Organization.
