What an Employee Placement Slip Is and When it’s Used

An Employee Placement Slip documents a temporary or permanent assignment of an employee to a worksite, client location, or project. It records who is being placed, the placement location, start and end dates, job role or duties, pay rate or billing rate, reporting manager, and any special instructions or restrictions. Employers, staffing agencies, and human resources teams use this slip to create a consistent record for payroll, timekeeping, access control, and invoicing. The form supports onboarding, compliance checks, and later audits where duty location and assignment details must be verifiable.

Why a Clear Placement Slip Matters

A complete Employee Placement Slip reduces administrative errors, clarifies responsibilities, and supplies evidence needed for payroll, client billing, and regulatory reviews. It helps maintain consistent expectations between the employer, employee, and client while preserving a dated record of assignment terms.

Why a Clear Placement Slip Matters

Who Typically Prepares and Signs the Slip

Copies should be retained by the employer, the employee, and any contracting client to ensure consistent recordkeeping and faster resolution of later discrepancies.

  • HR and Staffing Coordinators who manage assignments and recordkeeping for payroll and billing.
  • Project Managers or Site Supervisors who confirm role, schedule, and on-site requirements.
  • Employees accepting placement who must acknowledge duties, dates, and any safety or confidentiality terms.

Key Elements to Include on a Professional Placement Slip

A well-structured Employee Placement Slip groups assignment details into clear sections so reviewers can quickly verify status, dates, compensation, compliance checks, and signatory authority.

Employee ID

Unique employee identifier linked to payroll and HR records; ensures correct personnel matching across systems.

Assignment Dates

Start and end dates in MM/DD/YYYY format; indicate whether assignment is ongoing or time-limited.

Location & Client

Exact worksite address and client name for access approval, invoicing, and tax nexus considerations.

Role and Duties

Short description of responsibilities, required certifications, and any safety or clearance levels.

Compensation/Billing

Hourly rate or bill rate, overtime rules, billing frequency, and responsible invoicing party.

Signatures

Employee signature, employer representative signature, and signature dates to confirm acceptance and authority.

Step-by-Step: Completing an Employee Placement Slip

Follow these steps to produce a consistent, auditable placement record that integrates with payroll and client billing systems.

Where to Send or File the Completed Slip

Routing depends on organizational policy and whether the placement is internal, third-party client-facing, or for a government contract.

  • HR File: Retain original in the employee’s HR file for payroll and benefits reconciliation.
  • Payroll Team: Send to payroll to set rates and time-tracking rules.
  • Client Contact: Provide a client-facing copy if required by contract for on-site access or invoicing.
  • Compliance Archive: Store a secure copy for audits, tax compliance, or OSHA/contractor safety reviews.
