What the Employment Employer Request Is

An Employment Employer Request is a standardized document employers use to request employment-related information about a current or former employee from another employer or verification provider. It typically asks for dates of employment, job title, compensation history, rehire eligibility, and reason for separation. Organizations use it for background checks, hiring, credentialing, and loan underwriting. Responses form part of personnel files and decision records. The form may be delivered by mail, fax, secure portal, or eSignature platform and should comply with applicable privacy and consent laws.

Why a Standardized Request Matters

Use an Employment Employer Request to obtain verified employment facts that support hiring, lending, and credentialing decisions while establishing an audit trail. A standardized request reduces follow-up queries, ensures consistent data collection, and helps demonstrate compliance with consent and recordkeeping requirements.

Why a Standardized Request Matters

Who Typically Issues or Responds to These Requests

Employers, HR teams, background screening vendors, and lenders commonly issue Employment Employer Requests when verifying candidate work histories.

  • HR managers conducting pre-employment checks and reference verifications for compliance.
  • Hiring managers confirming job titles, duties, and dates for onboarding.
  • Lenders and credentialing bodies verifying employment history for underwriting decisions.

Recipients should document consent and retention to match internal policies and legal obligations and to support audit readiness.

Stepwise Process for Preparing and Sending the Request

Follow this sequence to prepare, authorize, and send the Employment Employer Request securely using appropriate consent and recordkeeping.

  • 01
Prepare: Gather employee identifiers and supporting documents.
  • 02
Obtain Consent: Secure written consent or electronic authorization to release information.
  • 03
Send Securely: Use encrypted email, secure portal, or eSignature.

Common Preparation Pitfalls to Avoid

  • Incomplete or inconsistent identifiers, such as name variations, missing SSN digits, or different date formats, lead to verification delays and require additional documentation requests.
  • Sending full Social Security Numbers over unsecured email creates significant data breach risk; follow data minimization and encryption practices or transmit only the last four digits when possible.
  • Using nonstandard templates or ambiguous questions produces inconsistent responses and may require manual follow-up, increasing processing time and attracting legal scrutiny.
  • Failing to document employee consent or retention policies complicates audits and may violate ESIGN, state privacy laws, or industry-specific rules.

How the Verification Flow Typically Works

Typical routing and response process for Employment Employer Requests when delivered electronically or by mail.

  • Upload Document: Attach a completed request template
  • Assign Signer: Designate HR contact and signer

Essential Parts of a Professional Request

A professional Employment Employer Request includes clear identifiers, explicit consent language, specific verification items, and a signed authorization to release information.

Header and purpose

State requester name, organization, purpose of the verification, and contact details. Clear purpose limits scope and supports lawful processing under privacy rules and employer policies.

Employee identifiers

Include full legal name, date of birth, last four SSN digits or TIN as required, employee ID, and address to ensure accurate matching and avoid false matches.

Verification items

List specific facts to confirm: dates of employment, job title, salary or compensation range, separation reason, and rehire eligibility. Avoid open-ended or ambiguous questions.

Consent statement

Provide a clear release authorizing the former employer to disclose information, specify electronic consent where used, and note the period during which consent remains valid.

Signature block

Require printed name, title, organization, signature, and date. For electronic signatures, capture authentication method and attach the audit trail for evidentiary purposes.

Attachments and proof

Attach supporting documents such as W-2s or pay stubs when permitted; mark documents confidential and include instructions for retention and secure disposal.

Practical Tips to Improve Accuracy and Speed

Best practices improve accuracy, compliance, and turnaround time for Employment Employer Requests when used consistently.

Standardize templates and required fields
Use a single approved template across the organization with mandatory fields to reduce omissions. Employ field validation for dates and phone numbers and include conditional fields for industry-specific data.
Use secure transmission and minimum data
Transmit requests and responses over encrypted channels only. Send the minimum personally identifiable information necessary for verification, redact full SSNs where possible, and log access for audit purposes.
Document consent and retention policies
Obtain and record explicit consent prior to requesting records from third parties. Align retention schedules to applicable rules such as I-9 and IRS timelines, and ensure secure archival procedures.
Train staff on legal exceptions
Educate HR and hiring managers on documents excluded from electronic execution, state-specific notarization rules, and privacy safeguards. Maintain a checklist for when attorney review or notarization is required.
