What an Endorsement Removal Request Is and When it Applies

An Endorsement Removal Request is a written instruction asking a party to remove or cancel a previously recorded or attached endorsement from a contract, insurance policy, title instrument, or financial document. The request documents who asked for removal, why the endorsement should be removed, the instrument or policy details, and the authority for the change. Organizations use this form to preserve an audit trail, confirm party consent, and create an actionable record for carriers, lenders, title companies, or contract counterparties before the endorsement is revoked or administratively removed.

Why a Clear Removal Request Matters

A formal request reduces ambiguity, creates a written record of consent or instruction, and protects parties from unintended liability or title defects. It supports audit trails and compliance with electronic signature laws such as the ESIGN Act (15 U.S.C. §7001) and UETA where applicable.

Why a Clear Removal Request Matters

Who typically prepares or receives this request

Common users include insurers, title and escrow companies, lenders, risk teams, legal departments, and individual policyholders or insureds.

  • Insurers and underwriters with delegated endorsement authority for policy adjustments and terminations.
  • Title companies and escrow agents managing recorded endorsements tied to property transactions.
  • Lenders and servicers coordinating removal of endorsements that affect loan collateral or security interests.

The form is also used by corporate legal teams and third-party administrators to document approvals and maintain chain-of-record for downstream processing.
