What the Equipment Laptop Issuance Form Is

An Equipment Laptop Issuance Form documents the assignment and return terms for a company-owned laptop. It records device identifiers (asset tag, serial number), condition at issuance, assigned user, department, and any permitted personal use. The form typically includes security responsibilities, software licensing notes, return instructions on separation, and an acknowledgement for employee signature. Organizations use it to maintain inventory accuracy, assign financial responsibility, and document acceptance of IT policies and acceptable use requirements.

Why a Formal Issuance Form Matters

Using a standardized Equipment Laptop Issuance Form reduces asset loss, clarifies liability, and creates an auditable record for security and accounting. It ensures employees accept device care, encryption, and return obligations and supports compliance during audits and incident investigations.

Why a Formal Issuance Form Matters

Who Prepares and Signs This Form

Final signatures usually include the employee receiving the laptop and an IT authorizing staff member to confirm issuance.

  • HR and People Ops: Records employee acknowledgement, links issuance to hire/termination records.
  • IT / Asset Management: Verifies device identifiers, installs controls, and tracks warranty data.
  • Finance or Procurement: Records cost center, depreciation start date, and asset ledger entries.
