What the Equipment Replacement Form Is and when it applies

An Equipment Replacement Form documents the authorized removal and substitution of physical assets such as machinery, IT hardware, or medical equipment. It records the item being replaced, the replacement item's description, serial numbers, warranty transfers, responsible parties, effective date, and any financial adjustments. The form can be executed between internal departments, vendors, lessors, or customers and serves as the official record of the replacement transaction.
