Independent Contractor Agreement Between Escort Agency and Escort Agreement made on the _________________ (date), between ____________________ (Name of Escort) of ___________________________________________________________ _______________ (street address, city, county, state, zip code), referred to herein as Escort, and __________________________ (Name of Escort Agency), a corporation organized and existing under the laws of the state of ______________, with its principal office located at ____________________________________________________________________ __________ (street address, city, county, state, zip code), referred to herein as Agency. Whereas, Agency owns and operates an Escort Referral Business at the address set forth above, and Agency desires to contract with Escort to perform the hereinafter described services on behalf of Agency; Whereas, Escort agrees to perform these services for Agency under the terms and conditions set forth in this Agreement. Now, therefore, for and in consideration of the mutual covenants contained in this Agreement, and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties agree as follows: 1. Notification of Changes. Contractor's performance obligations. After the end of the contract period that begins on the (date), escort agrees to complete the assignments that are set forth in this agreement. An escort will be paid five percent commission for each escort who performs escort services for at least four (4), five thousand dollars (5,000) in receipts, regardless of how many escorts have performed escort services or what revenue generated in relation to escort services and receipt from their own referrals that may be attributed to escorts. 5.
