What an Esthetics Services Agreement Covers

An Esthetics Services Agreement is a written contract that defines the scope, timing, payment, and responsibilities for professional skin, beauty, or spa treatments between a licensed provider and a client. Typical topics addressed include services to be performed, pricing and deposits, client health disclosures and contraindications, scheduling and cancellation policy, liability and indemnity limits, privacy and recordkeeping, and any required consents for medical or cosmetic procedures. The agreement also clarifies post‑treatment care, product use, and dispute resolution to reduce ambiguity and limit regulatory risk for both parties.

Why using a written agreement matters

A clear Esthetics Services Agreement sets expectations, documents client consent, and reduces disputes by recording pricing, cancellations, and aftercare. It also helps providers meet professional licensing and consumer protection obligations while preserving evidence of disclosures and client instructions.

Why using a written agreement matters

Who typically completes this agreement

Completing the document before treatment reduces regulatory exposure and is a best practice for client safety and business recordkeeping.

  • Licensed estheticians and spas — Use to record services, pricing, contraindications, and aftercare; supports professional standards and local licensing inspections.
  • Medical esthetics clinics — Combine with medical history and informed‑consent addenda for injectables, lasers, or procedures that carry greater risk.
  • Individual clients and guardians — Sign to acknowledge risks, provide health information, and accept cancellation and refund terms.

Core elements to include in a professional agreement

A robust Esthetics Services Agreement bundles service detail, payment, safety and consent provisions, scheduling rules, liability limits, and dispute procedures into a single document.

Scope of Services

Describe specific treatments, product brands, session duration, and any agreed modifications so expectations are precise and measurable.

Pricing & Payment

State total cost, deposit amount, accepted payment methods, applicable taxes, and when additional fees apply (e.g., add‑ons, rescheduling).

Health Disclosures

Collect allergies, medications, pregnancies, recent procedures, and other contraindications; require client attestations to accuracy.

Cancellation Policy

Specify notice period for cancellations, deposit forfeiture rules, and reschedule windows to limit last‑minute revenue loss.

Liability & Indemnity

Include limitation of liability for ordinary risks, indemnities for client misrepresentation, and carveouts for gross negligence or willful misconduct.

Consent & Aftercare

Obtain written consent for the procedure, capture post‑treatment instructions, and record client understanding of expected outcomes and recovery.

Required information to collect and record

Client Name: Full legal name
Contact Details: Phone, email, address
Health History: Allergies/conditions
Treatment Details: Service type/date
Payment Terms: Deposit/price noted
Signatures: Client and provider

Step-by-step: completing the agreement

Follow a consistent sequence to capture accurate client information, consent, and payment terms before any treatment begins.

  • 01
Prepare form: Use current template with practice branding and licensing details.
  • 02
Collect health data: Ask client to disclose meds, allergies, and recent procedures.
  • 03
Agree payment: Confirm price, deposits, and any installment terms.
  • 04
Obtain signatures: Get client signature and date before service starts.

Configuring the agreement for online completion

Set up a digital workflow that enforces fields, collects consent, and preserves a tamper‑evident audit trail for each signed agreement.

Form Fields Make name, DOB, medical history, and signature required fields.
Validation Rules Apply MM/DD/YYYY checks and phone/email format validation.
Conditional Logic Show follow‑up health questions when certain answers are selected.
Signer Authentication Use email verification or SMS code for basic identity proofing.
Audit Trail Capture timestamps, IP addresses, and action history.

Typical routing and submission flow

Understand the end‑to‑end path from form creation through client signing to final record retention.

  • Create: Provider prepares agreement and sets required fields.
  • Send: Deliver via secure link or email to the client.
  • Sign: Client reviews, signs, and dates the agreement.
  • Store: System stores signed copy with audit trail and access controls.

Technical and compliance considerations for e‑submission

Ensure the selected service can produce a tamper‑evident signed record, meet any HIPAA obligations (if applicable), and integrate with your practice management tools.

  • File formats: PDF and DOCX supported
  • Integrations: Connects to CRM and cloud storage
  • Security: TLS and AES‑256 encryption

Common timing and deadline rules to include

Build clear deadlines into the agreement so clients and providers understand payment timing, notice periods, and refund windows.

Deposit Due:

State when deposit must be paid to hold appointment.

Cancellation Notice:

Specify required hours or days to avoid forfeiting deposit.

Reschedule Window:

Define how far in advance a reschedule is allowed.

Refund Period:

List conditions and timeframe for refunds.

Follow-Up Care:

Provide window for post‑treatment check or complaint submission.

Common mistakes to avoid when preparing the agreement

  • Using vague service descriptions that leave room for differing expectations, causing client dissatisfaction or disputes.
  • Failing to collect essential health disclosures, which can increase the risk of adverse events and expose the provider to liability.
  • Relying on unsigned or improperly executed forms; unsigned consent or missing dates can weaken enforcement in disputes.
  • Not aligning cancellation or refund language with payment processors and state consumer protection rules, creating compliance problems.

Risks and potential consequences of errors

Service Disputes: Refunds or reputational harm
Licensing Violation: Fines or disciplinary action
HIPAA Breach: Civil penalties and remediation
Invalid Consent: Limits liability protection
Payment Chargebacks: Merchant fees and losses
Notary Errors: May invalidate certain acknowledgements
