What an Event Camp Statement Is and when to use it

An Event Camp Statement is a written participant declaration used by organizers to document attendee information, permissions, and acknowledgments for a camp, conference, or multi-day event. It commonly combines participant identification, emergency contacts, medical disclosures, parental or guardian consent for minors, liability and media-release language, and a signature block. The form creates a clear administrative and legal record for organizers and helps confirm that attendees understand rules, risks, and any special requirements before participating in activities.

Why an organized Event Camp Statement matters

A clear Event Camp Statement reduces operational risk, documents consent for minors, and centralizes emergency and medical information so staff can act quickly and consistently.

Why an organized Event Camp Statement matters

Who completes and relies on an Event Camp Statement

Organizations and individuals responsible for safety and compliance complete or collect these statements before the event begins.

  • Camp directors and program managers who must verify participant eligibility, consent, and emergency details prior to activities.
  • School administrators and youth program coordinators collecting parental consent and medical disclosures for minors attending field programs.
  • Event medical staff and onsite first responders who rely on emergency contact, allergy, and medication information during incidents.

Maintain copies for administrative review, emergency access, and legal recordkeeping according to applicable retention rules.

Typical signers and their roles

Camp Director

The director or authorized organizer signs as the recipient and keeper of records, confirming that procedures were explained and that the statement is stored in program files for the event period and any required retention window.

Parent/Guardian

For minors, a parent or legal guardian signs to provide consent for participation, medical treatment authorization, and media release when included; their signature establishes legal authority and contact chain for emergencies.

Essential components to include in a professional Event Camp Statement

Design the statement to be concise but complete; include identifiable fields, consent language, emergency details, and a clear signature area to avoid later disputes.

Participant ID

Full legal name, date of birth, and government ID or school ID number where applicable to ensure correct identification and to match medical or behavioral records.

Emergency Contacts

Primary and secondary contact names, relationship, and phone numbers; list daytime and evening numbers and any authorized alternate pickup persons for minors.

Medical Details

Allergies, chronic conditions, medications, and treating physician contact; note special instructions for on-site medication administration or restrictions.

Consent and Release

Clear, specific consent language for participation and treatment; if a waiver is desired, state risks plainly and confirm the signer understands and accepts them.

Media Release

Optional clause authorizing use of photographs or video; specify scope, duration, and whether minors are included. Keep opt-out options where required.

Signature Block

Signature, printed name, relationship to participant (if signing for a minor), and MM/DD/YYYY date; include witness or notary lines if legally required.

Common preparation mistakes to avoid

  • Using vague consent language that fails to specify activities covered, making enforcement or medical decisions harder.
  • Omitting emergency contact alternatives, which can delay urgent decisions if the primary contact is unreachable.
  • Leaving medical or medication fields optional when they should be mandatory for participant safety and staff planning.
  • Failing to align signature method with state rules or organizational policy, creating potential validity questions.

Liability Exposure: Increased legal risk for organizers
Medical Delay: Treatment may be delayed without details
