Medical Treatment Authorization: In the event of illness or injury to the participant while under the care or supervision of the Organizer, and if I cannot be reached in a timely manner, I authorize the Organizer and its agents to secure emergency medical and surgical treatment as may be deemed necessary. I agree to be financially responsible for such treatment and to provide medical insurance information above.

Assumption of Risk and Release: I acknowledge that participation in the event involves inherent risks. On behalf of the participant and myself, I voluntarily assume all such risks and hereby release, waive, covenant not to sue, and discharge the Organizer, its officers, employees, volunteers, agents, and affiliates (collectively, the Released Parties) from any and all liability, claims, demands, actions, or causes of action whatsoever for any loss, damage, injury, or death arising out of or related to the participant's attendance or participation, except to the extent such loss, damage, injury, or death is caused by the gross negligence or willful misconduct of the Released Parties.

Indemnification: I agree to indemnify, defend and hold harmless the Released Parties from and against any and all claims, liabilities, losses, damages, costs and expenses (including reasonable attorneys' fees) arising out of the participant's actions or omissions during the event.
