What the Exercise Participation Request Is

An Exercise Participation Request is a written or electronic form used to document an individual’s request to participate in a planned exercise, drill, or training activity. It captures participant identity, emergency contact details, medical or accommodation needs, consent for activity terms, and acknowledgment of applicable risks. Organizations use these requests to confirm eligibility, schedule participants, track headcounts, and comply with internal safety or regulatory requirements. The form can be issued as a paper document or as an electronically fillable record with an audit trail for signature and retention purposes.

Why an Exercise Participation Request Matters

Collecting a completed Exercise Participation Request protects organizers and participants by documenting consent, medical disclosures, and acceptance of terms, while creating a reliable record for scheduling and liability management.

Why an Exercise Participation Request Matters

Who typically completes an Exercise Participation Request
