What the Expense Authorization Consent Form Is

An Expense Authorization Consent Form documents pre-approval for planned spending by an individual or department on behalf of an organization. It names the requester and approver, specifies purpose, amount, budget code, permitted vendors and dates, and records explicit consent to payment or reimbursement. Organizations use it to control spending, establish accountability, and create a searchable legal record. When completed digitally, the form should preserve intent, consent, and an audit trail to meet ESIGN and UETA standards for enforceability across most U.S. jurisdictions.

Why Use an Expense Authorization Consent Form

A standardized consent form reduces unauthorized spending, improves budget tracking, creates a defensible audit trail, and speeds approvals. It clarifies duties, reduces disputes over reimbursements, and supports internal controls and external audits without changing substantive approval policies.

Why Use an Expense Authorization Consent Form

Who Typically Completes or Signs This Form

Common participants include the employee requesting funds, the approving manager, and finance or accounts-payable staff who process payment or reimbursement.

  • Employee requester — Submits complete justification, estimated amount, and supporting documentation for approval.
  • Approving manager — Verifies budget availability, project alignment, and compliance with spending limits before signing.
  • Finance or AP reviewer — Confirms receipts, posts accounting codes, and issues payment or reimbursement following approval.

Role clarity in the form helps routing, enforces limits, and ensures that approvals align with delegation-of-authority policies.

Key Signer Roles and Authority

Finance Manager

The Finance Manager enforces budget controls and verifies that the requested expense fits chart-of-accounts rules. They review supporting documentation, confirm approvals meet policy thresholds, and are responsible for final disbursement authorization or routing to accounts payable.

Department Head
