What the Expense Reimbursement Credit Card form is and how it’s used

An Expense Reimbursement Credit Card form documents charges incurred on a corporate or organizational credit card that an employee or contractor seeks to have reimbursed. It typically identifies the cardholder, merchant, date, business purpose, expense category, and amount, and attaches supporting receipts. Organizations use the form to authorize payment, ensure policy compliance, and create an auditable trail for accounting, payroll and tax reporting. Completed forms feed accounts payable workflows and may be retained for tax and audit purposes under applicable federal and state retention rules.

Why a clear Expense Reimbursement Credit Card form matters

A complete, consistent form reduces disputes, speeds reimbursement, and creates a defensible audit record for accounting, tax, and internal control reviews. Accurate fields minimize withholding or reporting errors and support downstream approvals and ledger entries.

Use structured categories and GL codes
Predefined categories reduce coding errors, speed reconciliation, and improve reporting accuracy; align categories with chart of accounts and train submitters on selection.
Require receipts over a threshold
Set a sensible receipt requirement threshold (for example, any claim over $25) to balance administrative overhead and fraud prevention.
Attach receipts at submission
Mandate receipt attachments in the digital form and automate verification where possible to eliminate follow-up requests and accelerate payment.
Preserve the audit trail
Keep the signed document, approval timestamps, and IP/authentication details as part of the archived record for audit and tax purposes.
