What the Experience Verification Declaration Is

An Experience Verification Declaration is a formal, signed statement that documents an individual's work history, duties, dates of employment, and relevant qualifications for licensing, credentialing, or employment verification. It is typically completed by a current or former employer, supervisor, or authorized representative and used by professional boards, background screening providers, human resources departments, or immigration authorities. When executed correctly this declaration provides verifiable evidence of experience and can be submitted in paper or electronic form, with electronic execution governed by federal and state e-signature laws such as the ESIGN Act and UETA.

Why a Clear, Verified Declaration Matters

Use this declaration to establish documented proof of professional experience for licensing, hiring, immigration, or credentialing reviews. It reduces back-and-forth verification requests and creates a clear record that supports decision-making while meeting legal standards for electronic signature and record retention.

Why a Clear, Verified Declaration Matters

Who Prepares and Who Receives These Declarations

Employers, licensing boards, HR teams, immigration agencies, and background screening firms commonly prepare or request this declaration to confirm an individual's work history and duties.

  • Current or former supervisors verifying roles, responsibilities, and dates of employment.
  • Human resources staff confirming job titles, pay grade, and employment status for records.
  • Professional licensing boards requiring sworn experience statements for certification or license renewal.

Signed declarations are frequently stored in personnel files, attached to credential applications, or provided directly to third parties that require verified experience.

Core Sections to Include in a Professional Declaration

Core sections of an Experience Verification Declaration outline parties, employment details, verification statements, supporting attachments, signature blocks, and authorization for record release.

Parties

Identify the declarant and subject: full legal names, business titles, company names, and contact information to establish who attests to the experience and how to follow up.

Employment Details

List employer name, job title(s), precise start and end dates (MM/DD/YYYY), location, including supervisory scope, tools used, and measurable outcomes where applicable.

Verification Statement

A clear, affirmative declaration that the signer personally verified the listed experience or had access to reliable records, and indicate whether verification was based on personnel files or personal recollection.

Supporting Documents

Attach pay stubs, performance evaluations, offer letters, project records, or other documents that corroborate dates, duties, and accomplishments referenced in the declaration, and redact sensitive personal identifiers where required.

Signature Block

Provide printed name, title, date (MM/DD/YYYY), and an explicit signature, and include contact information for verification follow-up such as direct phone or email. If an authorized representative signs, state their authority and capacity.

Consent & Release

Include a limited release authorizing the verifier to disclose employment information to the requesting party and note duration of consent or revocation method if applicable.

  • Failure to include verifier authority or contact information prevents third parties from confirming authenticity and increases audit risk.
  • Using informal signature methods without a clear audit trail undermines evidentiary value for licensing or immigration reviews.
