What the Experience Verification Form Is

The Experience Verification Form is a standardized document used to confirm an individual's prior employment, roles, dates of service, duties, and supervisor verification for hiring, licensing, contracting, or credentialing purposes. It collects verifier contact details, job titles, start and end dates, and a statement confirming accuracy. Organizations use the form to support background checks, professional licensing applications, and contractor evaluations; it creates an auditable record that supports consistent decisions and reduces reliance on informal references.

Why an Experience Verification Form Matters

A clear Experience Verification Form streamlines vetting, reduces disputes, and creates an auditable record for compliance. For electronic use it meets legal standards under the ESIGN Act (15 U.S.C. ch. 96) and UETA where adopted, preserving enforceability for interstate and intrastate transactions.
