Common Mistakes to Avoid

  • Incomplete scope descriptions that leave security teams uncertain which zones to provision.
  • Mismatched or abbreviated names that prevent identity verification and delay badge issuance.
  • Missing approver information or signatures that cause routing backlogs and manual follow-up.
  • Attaching low-quality scans or wrong documents that require re-submission and slow processing.

Risks and Consequences of Incorrect or Missing Information

Unauthorized access: Liability and security incidents
Insurance exposure: Claims denial risk
Operational disruption: Delayed maintenance or emergency response
Regulatory noncompliance: Fines or corrective actions

Practical Tips for Accurate and Efficient Completion

Adopt these practices to reduce errors, speed processing, and maintain an auditable trail.

Verify custodian identity and authority before submission
Confirm names against government ID or HR records, document the verification method, and record the approver’s name; this avoids later challenges to access decisions and supports auditability.
Use clear, specific scope descriptions for access and duties
List exact rooms, systems, or assets included in custody and any exclusionary clauses; specificity prevents scope creep and ensures security teams provision access correctly.
Retain signed records in a secure, searchable archive
Store the executed form with an immutable audit trail, index by facility and custodian, and ensure the record retention schedule aligns with legal and policy requirements.
Standardize the form and automate routing
Use a single version of the form across locations, automate approval chains, and include conditional fields to reduce manual reviews and processing time.
