Service Provider shall treat all family information, medical records, court orders, and other personally identifying information provided by Client as confidential material and shall not disclose such information except: (a) with Client's written consent; (b) to contracted persons who have a legitimate need to know in order to perform services under this Agreement and who are bound by similar confidentiality obligations; or (c) as required by law or court order. Service Provider will implement reasonable administrative and technical safeguards to protect confidential information.

11. GOVERNING LAW

This Agreement, and any dispute arising out of or related to it, shall be governed by the laws of the State of without regard to its conflicts of law principles.

12. ENTIRE AGREEMENT

This document, including any attachments or addenda signed by the parties, constitutes the entire agreement between Client and Service Provider with respect to the subject matter herein and supersedes all prior oral or written representations, negotiations, and agreements. Any modification must be in writing and signed by both parties.

13. ACKNOWLEDGMENT AND CERTIFICATION

By signing below, Client certifies that the information provided on this form is true and accurate to the best of Client's knowledge and that Client has the authority to provide such information for household members.
