What the Fatigue Management Assessment Form Is

The Fatigue Management Assessment Form is a standardized workplace document used to record employee-reported sleep and alertness data, work schedules, task exposures, and mitigation steps. Organizations use it to evaluate fatigue risk, document decisions about fit-for-duty or modified duties, and create a record for follow-up reviews. When executed electronically, the form can include an audit trail and signer attribution consistent with U.S. electronic signature law, enabling secure remote completion without requiring paper handling.

Why a Formal Assessment Matters for Safety and Compliance

A consistent Fatigue Management Assessment Form centralizes key data, reduces ambiguity in risk decisions, and documents controls for regulatory or internal review. Accurate records show due diligence in safety programs and support incident investigations.

Why a Formal Assessment Matters for Safety and Compliance

Typical Users and Signers for This Form

Primary users include safety managers, HR professionals, occupational health clinicians, and frontline supervisors responsible for assessing fatigue risk.

  • Safety Manager: Reviews aggregated assessments and tracks corrective actions.
  • Occupational Health Clinician: Evaluates medical factors and recommends accommodations.
  • Supervisor: Documents schedule, tasks, and immediate mitigations.

The form is often routed to multiple reviewers and retained with employee health or safety records per organizational policy.

Step-by-Step: Completing the Assessment

Complete the form in sequence to preserve context and ensure reviewers can act on documented risks.

  • 01
    1. Enter identity: Fill employee name and ID before other fields.
  • 02
    2. Record schedule: Provide recent duty hours and shift rotation.
  • 03
    3. Describe symptoms: Note subjective sleepiness and observed impairment.
  • 04
    4. Sign and route: Sign, date, and send to supervisor and health reviewer.

Typical Routing and Decision Flow

A predictable routing sequence helps ensure assessments are reviewed and mitigation actions are implemented promptly.

  • Submit: Employee or supervisor completes form and submits.
  • Supervisor review: Supervisor evaluates tasks and applies immediate controls.
  • Health review: Occupational health assesses medical risk and recommendations.
  • Recordkeeping: Final form stored in safety or health record systems.

Recommended Digital Workflow Settings

Configure form fields and routing to match your organizational controls and audit requirements.

Field Configuration
Identity Field Required; read-only after sign
Schedule Section Conditional visibility for night shifts
Medical Notes Restricted to occupational health role
Signature Method Email or stronger auth (SMS/KBA) as needed

Technical Considerations for eSubmission

Verify platform capabilities before rolling out electronic assessments to ensure authentication, storage, and integrations meet policy requirements.

  • Authentication: Support email, SMS, or stronger methods per risk level.
  • File formats: Accept PDF and DOCX for consistent archiving.
  • Integrations: Connect to HRIS, EHR, or safety systems where required.

Ensure the chosen platform supports audit trails, role-based access, and any required compliance controls before storing sensitive health or safety records.

Core Sections to Include in a Professional Form

A complete Fatigue Management Assessment Form contains identification, exposure details, symptom data, and a documented mitigation plan to support safe decisions.

Identification

Employee name, job title, ID, and contact information. Clear identity data prevents misattribution and supports subsequent follow-up or work restrictions.

Work Schedule

Recent shifts, cumulative hours, and rotation pattern. Accurate schedule data identifies circadian disruption and cumulative fatigue risk.

Sleep and Symptoms

Self-reported sleep hours, sleep quality, and observed impairment indicators. Documenting symptoms provides evidence for temporary work adjustments or medical referral.

Task Exposure

List of safety-sensitive tasks performed and exposure frequency. This informs urgency and the selection of engineering or administrative controls.

Mitigation Plan

Specific, time-bound controls (rest breaks, reassignment, transport home). Concrete actions reduce ambiguity and improve compliance tracking.

Review & Follow-up

Signatures, reviewer notes, and scheduled reassessment date. A documented follow-up loop reduces recurrence and demonstrates due diligence.

Security and Compliance Controls to Note

Encryption in transit: TLS 1.2/1.3
Encryption at rest: AES-256
Access reports: Audit trail available
HIPAA readiness: BAA required for PHI
Legal framework: ESIGN and UETA compliant
Certifications: SOC 2 Type II, ISO 27001

Consequences of Incomplete or Incorrect Assessments

Workplace incidents: Increased injury and liability risk
Regulatory scrutiny: OSHA citations or enforcement actions
Workers' compensation: Greater claim exposure and costs
Legal claims: Negligence lawsuits and damages
Invalid documentation: Missing signatures may void decisions
Data breach: Privacy fines and remediation costs

Common Mistakes to Avoid

  • Failing to capture recent shift patterns or off-duty sleep history, which undermines the assessment of cumulative fatigue.
  • Using vague mitigation language like 'monitor' without specifying who, when, and how the action will be carried out.
  • Omitting required reviewer signatures or relying on initials when a dated signature is needed for legal defensibility.
  • Storing health-related notes in unsecured locations or systems without role-based access and audit logging.

Timing Guidelines and Internal Deadlines

Establish internal timelines so assessments are completed and reviewed quickly after identification of fatigue risk or an incident.

Initial assessment timing:

Complete on discovery or at shift end, ideally within 24 hours.

Post-incident review:

Document and route assessment within 72 hours after an incident.

Periodic reassessment:

Schedule routine reassessments at least annually or per policy.

Follow-up action deadline:

Implement short-term controls within 24–72 hours of review.

Record access window:

Provide reviewers access immediately after signing for audit.

Processing Milestones from Submission to Closeout

Track these milestones to move each assessment from submission to documented resolution efficiently.

01

Submission

Employee or supervisor submits completed assessment with all fields filled.

02

Supervisor decision

Supervisor documents immediate control or reassignment decisions.

03

Clinical review

Occupational health evaluates need for medical action or restrictions.

04

Closeout

Document actions taken and schedule required follow-up.

Practical Examples of Form Use

Real-world scenarios show how the form documents risk and supports decisions across contexts.

Example 1

A night-shift nurse reports 4 hours of sleep

  • Supervisor documents temporary reassignment from medication rounds
  • Occupational health recommends two weeks of adjusted scheduling and documents monitoring steps for handoff continuity and patient safety.

Example 2

A road crew operator shows signs of drowsiness after extended shifts

  • Supervisor removes operator from heavy equipment tasks immediately
  • A mitigation plan schedules rest, temporary reassignment, and a follow-up assessment within 72 hours to verify recovery.

Practical Tips for Accurate and Efficient Completion

Adopt these practices to improve data quality, reduce rework, and strengthen the record for audits or investigations.

Use consistent formats
Require MM/DD/YYYY for dates and 24-hour or clearly labeled shift times to avoid ambiguity during reviews and when aggregating data.
Limit PHI exposure
Only capture necessary health details and store them under access controls; separate clinical notes from operational summaries.
Automate routing
Set automatic routing rules so supervisors and occupational health receive copies immediately after submission to reduce delays.
Document outcomes
Record the exact mitigation applied, who authorized it, and the planned reassessment date to close the loop.
