What the FDD Application Form Is and When It Starts the Process

The FDD Application Form is the standard questionnaire prospective franchisees complete to request a Franchise Disclosure Document (FDD) from a franchisor and to begin the franchise evaluation process. It collects applicant identification, business background, financial disclosures, and requested franchise territory details. Franchisors use the form to assess suitability, verify experience and financial capacity, and to determine whether to provide a complete FDD. This document often starts the due diligence timeline and triggers delivery obligations under federal and state franchise laws.

Why an Accurate Application Matters

Completing the FDD Application Form accurately speeds review, ensures compliance with disclosure timing, and helps franchisors evaluate candidates efficiently. A correct application reduces follow-up requests, supports clearer franchise offering disclosure, and establishes an auditable record for regulatory compliance.

Why an Accurate Application Matters

Who Handles and Completes the Form

Prospective franchisees and franchisor intake teams use the FDD Application Form to gather candidate details and start the disclosure review process.

  • Prospective franchisee: submits personal, business, and financial information for franchisor evaluation.
  • Franchisor intake team: reviews suitability, confirms territory availability, and requests missing documents.
  • Legal and compliance staff: verify disclosures meet federal and state franchise regulations before FDD delivery.

Accurate completion reduces processing time, limits regulatory risk, and preserves a clear audit trail for future reference.

Typical Signers and Their Roles
