What the Fellowship Program Request Form Is

The Fellowship Program Request Form is a structured application used by institutions to request approval, funding, or placement for individual fellowship candidates. It collects applicant identity, project summary, supervisor endorsements, budget estimates, and compliance attestations. Organizations use the form to document approvals, route internal signoffs, and create a permanent record of the fellowship award or placement. When completed electronically it must meet e-signature legal tests under the ESIGN Act (15 U.S.C. ch. 96) and state UETA law where applicable to be enforceable.

Why a Standard Request Form Matters

A standardized Fellowship Program Request Form ensures consistent review, creates an auditable approval trail, reduces duplicate requests, and clarifies budget and supervision responsibilities for reviewers and finance teams.

Essential Sections to Include in a Professional Request Form

Project Description

Require a concise summary, objectives, timeline, and expected deliverables so reviewers can assess fit with program goals and resource needs.

Budget and Funding

Include itemized cost lines, total funding requested, and confirmed funding source to allow finance teams to validate availability and compliance.

Approvals and Endorsements

Provide designated fields for supervisor, department head, and grants office signatures to document institutional consent and accountability.

Compliance Attestations

Add checkboxes for conflict of interest, human subjects, export controls, or HIPAA considerations where applicable to flag required reviews.

Signature and Date

Reserve a final signature block with printed name, role, and date; clearly indicate whether electronic signatures are accepted under institution policy.

Common Preparation Mistakes to Avoid

  • Submitting drafts with missing attachments that reviewers expect, causing repeated back-and-forth and review delays.
  • Using informal or abbreviated names that do not match payroll or grant systems, which can block payments or approvals.

  • Relying on image-based signatures without an audit trail when legal or institutional policy requires stronger authentication methods.
