What the FEMA Disaster Assistance Form Is

The FEMA Disaster Assistance Form is the primary application used by individuals and households to request federal help after a presidentially declared disaster. It collects contact and identification data, damage descriptions, insurance information, and direct-deposit banking details so FEMA can assess eligibility for housing assistance, home repairs, and other support programs. The form is accepted online, by phone, and at disaster recovery centers; accurate, complete responses reduce follow-up requests and speed eligibility determinations and payments under federal disaster assistance rules.

Why This Form Matters for Recovery and Recordkeeping

The FEMA Disaster Assistance Form centralizes eligibility screening for individual assistance after disasters. Submitting accurate information helps applicants access temporary housing, repair grants, and disaster-related programs while documenting losses for insurance or other relief coordination and improving timeliness of inspections and payment.

Why This Form Matters for Recovery and Recordkeeping

Who Typically Completes the FEMA Disaster Assistance Form

Applicants, household representatives, and agency caseworkers use the FEMA Disaster Assistance Form to request and assess emergency individual assistance following a declared disaster.

  • Disaster survivors filing for housing assistance, home repairs, or other individual aid
  • Authorized representatives or agents submitting on behalf of affected households
  • Nonprofit or government caseworkers assisting vulnerable populations with registration

Local disaster recovery partners and nonprofit caseworkers also review submissions to confirm eligibility, verify documentation, and coordinate inspections and payments.

Who May Sign or Submit

Applicant

An adult household member who experienced the loss or damage. The applicant provides identification, contact details, SSN/TIN or alternative ID, damage description, and banking information for direct deposit. The named applicant certifies the truthfulness of submitted information and may be held responsible for repayment if assistance is obtained fraudulently.

Authorized Representative

A person legally authorized to act for the applicant, such as a power of attorney, attorney, or designated caseworker. The representative must provide proof of authorization and will sign or attest to the information on the applicant’s behalf when required by FEMA program rules.

Core Sections You’ll Find on the Form

The FEMA Disaster Assistance Form is organized to capture identity, loss details, insurance coordination, and financial routing. Each section supports eligibility review and payment processing.

Identification

Collects legal name, date of birth, social security number or tax identification, and primary contact details to verify applicant identity and match records with FEMA systems.

Damage Description
