What the Field Review Declaration Form Is

The Field Review Declaration Form records observations, conditions, and attestations made during an on-site inspection or field visit. It captures identity and contact details for the declarant, the date and location of the visit, a structured findings section, and the declarant's signature and date. Organizations use this form to document compliance checks, property condition reviews, insurance inspections, and lender site verifications. When completed accurately it becomes part of the official file and supports dispute resolution, audit trails, and regulatory compliance when retained according to applicable retention rules.

Why a Formal Field Review Declaration Matters

A Field Review Declaration creates an auditable, dated record of onsite findings and the declarant's attestations. It reduces ambiguity about observed conditions, clarifies who made the observations and when, and supports compliance with internal and external audit requirements including government and insurance reviews.

Why a Formal Field Review Declaration Matters

Typical Users and Signers

The form is used by organizations that require verifiable field observations and formal attestation from authorized staff or third-party inspectors.

  • Property managers and leasing agents responsible for condition reports and lease-related inspections.
  • Insurance adjusters and examiners documenting damage, cause, and recommended next steps for claims.
  • Lenders, underwriters, and project reviewers confirming site conditions or completion milestones.

Use the form when an on-site declaration is required by policy, contract, or regulatory obligation; the signer should be someone with direct knowledge of the visit.

Core Elements of a Professional Field Review Declaration

A clear structure makes the form reliable and defensible: identify parties, capture visit specifics, present objective findings, attach evidence, include attestation language, and provide a signature block with authentication options.

Declarant Details

Full legal name, title, organization, and contact information for the person completing the review; establishes attribution and accountability for the observations recorded.

Site Identification

Exact site address, parcel or project identifier, and any contract reference numbers so the declaration links to the correct property or project file.

Observations Checklist

Structured checkboxes and short-answer fields for condition items, measured values, safety concerns, and specific observations to reduce free-text ambiguity.

Attachments

Space to reference attached photos, diagrams, measurements, and third-party reports; label each attachment and record file names or IDs.

Attestation Language

A short declaration that the information is true to the declarant's knowledge, with a statement about intent to sign and consequences of false statements.

Signature & Witness

Signature line with printed name, date, and optional witness or notary block when required by policy or jurisdictional rules.

Step-by-Step: Filling and Submitting the Form

Complete the form in four practical steps to ensure accuracy and a verifiable record.

  • 01
Prepare: Gather ID, contract numbers, and photo evidence before starting the form.

Configuring a Digital Workflow for the Form

Key settings help automate routing, evidence capture, and authentication in a digital workflow.

Conditional Fields Show witness/notary blocks only when jurisdiction or policy requires them.

Retention Trigger:

Consequences of an Incorrect or Incomplete Form

Invalid Report: May be rejected by reviewers
Contract Delay: Can postpone approvals or disbursements
Financial Exposure: May increase liability or audit risk
Claims Denial: Insurance claims might be contested
Regulatory Penalty: Noncompliance fines in regulated contexts
Evidence Gaps: Weakens position in disputes or litigation

Common Preparation and Submission Errors

  • Mismatched declarant name and ID caused delays when identity could not be verified against supporting documentation.
  • Incomplete site addresses or missing parcel numbers made it difficult to match the declaration to the correct property file.
  • Poorly labeled photos or missing timestamps reduced the evidentiary value of attachments in claims or audits.
  • Asking declarants to initial instead of sign when the form requires a formal signature created acceptance issues.
