FIELD TRIP PERMISSION AND WAIVER I, give permission for my child , age to leave for purposes of traveling in a car or on public transportation for any reason including normal conditions under which children are transported including but not limited to trips to parks, zoos, play centers, shopping and a variety of locations for educational and entertainment purposes, including any emergency situations. I give permission for said child to walk to and/or participate in activities geared for my child but away from above mentioned Childcare Provider under the supervision of or any authorized employees of .
