Termination of Authorization: This Authorization will remain in full force and effect until the Billing Company receives written notification of revocation at least 30 days prior to the next scheduled draft. Notification must include account holder name, account details, and signature.

By signing below, the Account Holder authorizes the Billing Company to initiate debit entries to the account or card indicated above and, if necessary, to initiate adjustments for any entries made in error. The Account Holder represents and warrants that they are an authorized signer on the specified account or card and have authority to execute this Authorization.

The Account Holder agrees to notify the Billing Company of any changes to account information or termination of this Authorization in writing at least 30 days prior to the next scheduled draft. The Billing Company reserves the right to suspend or cancel autodraft for returned items, suspected fraud, or other reasonable cause.

Returned payment fee: The Account Holder agrees to pay a fee for any returned or dishonored debit or charge as established by the Billing Company and permitted by law. Interest may accrue on overdue balances at the rate stated in the underlying service agreement or, if none, at a rate permitted by law.

The Account Holder acknowledges that this Authorization is governed by applicable rules of payment networks and clearinghouses and that the Billing Company may process entries in accordance with those rules. The Account Holder shall indemnify and hold harmless the Billing Company for claims arising from inaccurate information provided by the Account Holder or for any disputes arising from underlying goods or services.
