By signing below, the undersigned Payor certifies that all information provided on this form is true and complete to the best of their knowledge. The undersigned authorizes Issuer to process a one-time payment in the amount set forth in Total Due using the payment method selected above. This authorization is for a single payment only and does not constitute ongoing authorization for future charges unless expressly indicated in writing.

Payor agrees that payment not received by the Due Date shall bear a late fee equal to the lesser of 1.5% per month or the maximum amount permitted by law, and Issuer may pursue collection, including reasonable attorneys’ fees and costs. Issuer's failure to enforce any provision shall not be a waiver. This form and the rights and obligations hereunder shall be governed by the law of the jurisdiction of Issuer's principal place of business, without regard to conflict-of-law rules.

Disputes: To dispute any charge, Payor must notify Issuer in writing within ten (10) calendar days of receipt of the invoice setting forth the basis for the dispute and relevant documentation. Timely notification does not suspend Payor's obligation to pay undisputed amounts by the due date.
